超声造影技术在肾脏肿瘤诊断和鉴别诊断中的应用

摘要

目的 探讨超声造影技术在肾脏肿瘤诊断和鉴别诊断中的作用.方法 回顾性分析2013年10月至2015年10月于本院行超声造影的48例肾细胞癌患者的临床资料,观察超声造影和血流灌注特点,经病理检查证实后对各类细胞癌进行对比分析.结果 肾透明细胞癌患者超声造影呈现"快进慢出高增强"表现,即造影剂填充快速,早于周围正常肾脏组织,达峰时间快于周围正常肾脏组织;峰值强度低于乳头状肾细胞癌;而肾嫌色细胞癌和乳头状肾细胞癌患者造影剂填充慢于周围正常肾脏组织,峰值强度较低,消退强度也低于周围正常肾脏组织.乳头状肾细胞癌患者造影呈现"慢进低增强"表现,造影剂到达时间和达峰时间均短于周围正常肾脏组织.肾嫌色细胞癌患者造影呈现"慢进与增强"表现,造影剂填充慢于周围正常肾脏组织,达到峰值时约等于肾皮质.结论 超声造影技术在肾脏肿瘤的诊断和鉴别诊断中具有重要的临床应用价值.%Objective To investigate the role of contrast-enhanced ultrasound in the diagnosis and different-iation of renal tumors.Method The contrast-enhanced ultrasound dataof 48 patients with renal cell carcinoma in our hospital from October 2013 to October 2015 were retrospectively analyzed. The characteristics of contrast-enhanced ultrasound and blood perfusion were observed, and the results were compared.ResultThe contrast-enhanced ultrasound data of renal clear cell carcinoma showed "fast in, slow out and high performance enhancement", meaned that the contrast agent filled quickly, earlier than the surrounding normal tissue, the peak time was faster than the surrounding tissue, peak intensity was lower compared with renal papillary carcinoma. The contrast agent in chromophobe cell carcinoma and papillary cell carcinomafilled slower than the surrounding normal tissue, with low peak intensity, fading intensity was also lower than the surrounding normal tissue. Papillary cell carcinoma showed "slow in and low performance enhancement", the arrival time of contrast agent and the time to peak were lower than the surrounding normal tissue. Chromophobe cell carcinoma showed "slow in and equal performanle enhancement", the arrival time of contrast agent was slower than the surrounding trssue, peak intensity is approximately equal to the renal cortex.Conclusion Contrast enhanced ultrasound shows great value in the diagnosis and differentiation of renal tumors, and has the broad prospects of development.
