Objective To study the relationship of serum uric acid (UA) level with metabolic syndrome (MS) and chronic kidney disease (CKD) in elderly residents of Beijing communities.Methods A total of 1419 elderly subjects from 3 Beijing communities who underwent physical examination were included in this study.Their waist circumference,blood pressure,and serum levels of blood glucose,TC,TG,HDL-C,LDL-C,UA,urea nitrogen and creatinine were measured and their glomerular filtration rate (eGFR) was assayed.Relationship of serum UA level with MS and CKD was analyzed.Results The age was older,the SBP and DBP,the prevalence of smoking and the serum low HDL-C level were higher while the prevalence of obesity,the serum levels of TG and high LDL-C,the incidence of hyperuricemia and MS were lower in males than in females (P＜0.01).No significant difference was found in the prevalence of CKD between men and women (P=0.283).The serum UA level was positively related with the serum urea nitrogen and creatinine level (rmale=0.148,rfemale =0.247,P＜0.01;rmale =0.495,rfemale =0.514,P＜0.01) and was negatively related with eGFR in both males and females (rmale =-0.358,rfemale =-0.416,P＜0.01).Logistic regression analysis showed that age and hyperglycemia were the independent risk factors for CKD in both males and females.Conclusion Hyperuricemia is an important risk factor for CKD.However,MS is not related with CKD.%目的 横断面调查北京社区老年居民血清尿酸水平与代谢综合征(metabolic syndrome,MS)和慢性肾病(chronic kidney disease,CKD)的关系.方法 北京3个大型社区老年健康体检者1419例,采集腰围、血压、空腹血糖、TC、TG、HDL-C、LDL-C、尿酸、尿素、肌酐等指标,并根据公式估算肾小球滤过率(eGFR),分析不同性别血清尿酸与MS和CKD的关系.结果 男性年龄、收缩压、舒张压、吸烟、低HDL-C血症显著高于女性,而肥胖、高TG血症、高LDL-C血症、高尿酸血症、MS患病率显著低于女性,差异有统计学意义(P＜0.01).不同性别CKD患病率无明显差异(P=0.283).男性和女性尿酸与尿素、肌酐呈正相关(r男=0.148,r女=0.247,P＜0.01;r男=0.495,r女 =0.514,P＜0.01),与eGFR呈负相关(r男=-0.358,r女=-0.416,P＜0.01).logistic回归分析显示,年龄和高尿酸血症是不同性别CKD患病的独立危险因素.结论 高尿酸血症是CKD的重要危险因素,但MS与CKD无明确关系.
