Objectives:To explore the clinical effect of traditional Chinese medicine (TCM) differentiation combined with tamsulosin in the treatment of type Ⅲ chronic prostatitis (CP) with sexual dysfunction.Methods:By the random number table method,160 patients with CP and sexual dysfunction were divided into treatment group (n =80) and control group (n =80).The control group was given tamsulosin treatment,while the treatment group was additionally treated with TCM differentiation.Patients with CP were divided into the downward flow of damp-heat type,qi stagnation and blood stasis type,liver-kidney yin deficiency type and kidney-yang deficiency type,and they were given individualized treatment.The curative effect was evaluated,and the degree of sexual dysfunction was assessed by CP related sexual dysfunction scale (PSFI).The levels of serum inflammatory factors were determined.Results:The total effective rate of treatment in treatment group was significantly higher than that in control group (81.25 ％ vs 67.50％) (P ＜ 0.05).The recurrence rate in treatment group was significantly lower than that in control group during 6 months after treatment (3.75％ vs 11.25％) (P ＜ 0.05).There was no significant difference in the degree of sexual dysfunction between the two groups before treatment (P ＞ 0.05).After treatment,the degree of sexual dysfunction in two groups was obviously relieved (P ＜ 0.05),and the relief degree was more significant in treatment group than control group (P ＜ 0.05).There was no significant difference in the levels of inflammatory factors between the two groups before treatment (P ＞ 0.05).After treatment,the levels of serum inflammatory factors decreased significantly lower in treatment group than control group (P ＜ 0.05).Conclusion:The effect of TCM differentiation combined with tamsulosin is better than that of western medicine alone in the treatment of type Ⅲ CP.The combined treatment can significantly reduce inflammation and improve sexual function.%目的:探究中医辨证论治联合坦洛新治疗Ⅲ型慢性前列腺炎(CP)合并性功能障碍的临床效果.方法:160例CP合并性功能障碍的患者随机数表法分为治疗组(n=80)和对照组(n=80).对照组给予以坦洛新为主的西药治疗,治疗组在对照组治疗基础上联合中医辨证论治,将CP患者分为湿热下注型、气滞血瘀型、肝肾阴虚型、肾阳不足型4型,根据具体病例辨证采用个体化治疗.评定治疗疗效,采用CP相关的性功能障碍评分表(PSFI)评定性功能障碍程度,并测定血清炎症因子水平.结果:治疗组临床总有效率为81.25％,显著高于对照组的67.50％ (P ＜0.05),且治疗后6个月观察期内复发率为3.75％,明显低于对照组的11.25％ (P ＜0.05).两组治疗前性功能障碍程度分布比较无统计学意义(P＞0.05),治疗后治疗组、对照组性功能障碍程度均明显减轻,与治疗前相比有统计学意义(P＜0.05),且治疗组性功能障碍改善程度显著优于对照组(P＜0.05).两组治疗前各项炎症因子水平比较无统计学意义(P＞0.05),治疗后治疗组血清炎症水平下降幅度显著优于对照组(P＜0.05).结论:中医辨证论治联合坦洛新治疗Ⅲ型CP合并性功能障碍的临床疗效显著优于单纯西药治疗,对减轻机体炎症反应和改善性功能有重要价值.
