甲状腺癌中医辨证唾液蛋白质组诊断模型研究

         

摘要

Objective To investigate the difference of salivary protein mass spectrum between different traditional Chinese medicine (TCM) syndromes of thyroid cancer, and establish a diagnostic model to provide an objective base for TCM syndrome of thyroid cancer. Methods Salivary protein fingerprints of 49 thyroid cancer patients (23 cases of phlegm and blood stasis syndrome, and 26 cases of liver qi stagnation syndrome) were detected by SELDI-TOF-MS (surface enhanced laser desorption/ionization time-of-flight mass spectrometry). And a diagnostic model was established with the help of bioinformatics. Results Through the comparison of salivary protein fingerprints from the two groups, 29 protein peaks showed significant difference (P<0.01). M/Z 3 809.85, 3 110.04, 4 970.80 were selected to construct the optimal decision-tree model. In the test group, accuracy was 85.7% (42/49), sensitivity was 84.6% (22/26), and specificity was 86.9% (20/23). Conclusion A preliminary model for TCM syndrome differentiation of thyroid cancer-salivary protein fingerprint was established, which may provide a novel method and approach to the investigation of the essence of TCM syndromes.%目的 探讨甲状腺癌不同证型患者的唾液蛋白表达质谱的差异并构建诊断模型,为甲状腺癌的中医辨证分型提供客观依据.方法 应用表面增强激光解析电离飞行时间质谱(SELDI-TOF-MS)技术分别检测23例痰瘀互结型和26例肝郁气滞型甲状腺癌患者的唾液蛋白质指纹图谱,筛选出2组间特异性表达差异蛋白,并结合生物信息学方法 建立诊断模型.结果 通过2组唾液蛋白质指纹图谱数据比较,共获得29个有明显差异的蛋白峰(P<0.01),其中M/Z 3809.85、3 110.04、4 970.80被选择用于构建最佳决策树模型,该模型的测试组总准确率为85.7%(42/49),灵敏度为84.6%(22/26),特异性为86.9%(20/23).结论 初步建立了甲状腺癌的唾液蛋白质指纹图谱辨证模式,为中医证候实质研究提供了新的方法 和途径.
