Objective To investigate the clinical strategy and excision technique for resection of huge tumor in left upper abdomen. Methods From May 2005 to May 2014, the clinical data of 62 cases admitted from a huge tumor in patients with left upper quadrant were analyzed. the relationship among the amount of bleeding, the median operative time, postoperative complications were analyzed. The clinical features of a huge tumor of the left upper abdomen, surgical treatment strategies, operative skills were summarized. Results Sixty-two patients underwent radical combined organ resection of left upper abdominal tumor, for different surgical procedure, the median amount of bleeding was 250-600 ml, the median operative time was 110-170 min, 57 cases recovered smoothly after operation, There were 3 cases of pancreatic fistula, 2 cases of left upper abdominal effusion, 2 cases of left upper quadrant, intestinal adhesion, intestinal obstruction, and no patient died. Conclusions Mastering the clinical strategy and operation skills of resection of giant tumor in left upper abdomen can improve the radical resection rate and reduce the risk of operation.%目的 探讨左上腹巨大肿瘤切除术的临床策略及切除技巧.方法 回顾分析2005年5月至2014年5月62例左上腹巨大肿瘤患者的临床资料.分析患者术中中位出血量、中位手术时间、术后并发症,总结左上腹巨大肿瘤的临床特点、外科治疗策略及手术操作技巧.结果 本组62例患者均行左上腹肿瘤根治性联合脏器切除术,不同的手术方式的中位出血量为250~600 ml,中位手术时间110~170 min,其中57例术后恢复顺利,3例出现胰瘘,2例出现左上腹积液,2例左上腹小肠粘连肠梗阻,无死亡病例.结论 掌握左上腹巨大肿瘤切除术的临床策略及手术操作技巧,能明显提高肿瘤的根治性切除率,降低手术风险.
