体表孤立性神经鞘瘤与神经纤维瘤的超声诊断和鉴别诊断

         

摘要

目的:探讨体表孤立性神经鞘瘤与神经纤维瘤的超声诊断和鉴别诊断。方法统计分析2012-11—2014-03我院收治的116例体表孤立性良性神经源性肿瘤患者的临床资料。结果116例患者中,超声诊断表明神经鞘瘤55例,神经纤维瘤43例,脂肪瘤8例,皮脂腺囊肿4例,脂膜炎3例,腱鞘囊肿3例;病理诊断表明64例为神经鞘瘤,52例为神经纤维瘤。超声对神经鞘瘤诊断的符合率为70.9%,神经纤维瘤的符合率为65.1%,神经源性肿瘤的符合率为84.5%,误诊率为15.5%;体表孤立性神经鞘瘤和神经纤维瘤包膜的回声检出率差异无统计学意义(χ2=0.74,Ρ>0.05)。结论临床在运用超声鉴别诊断体表孤立性神经鞘瘤和神经纤维瘤的过程中可以依据二者与周围神经的关系。%Objective To investigate the ultrasound diagnosis and differential diagnosis of surface solitary schwannomas and neurofibromas.Methods The clinical data of 1 1 6 cases with benign surface isolated neurogenic tumors in our hospital from November 2012 to March 2014 were statistically analyzed.Results In 1 1 6 patients,ultrasonic diagnosis showed that 55 cases had schwannoma,43 cases had neurofibroma,8 cases had lipomas,4 cases had sebaceous cysts,3 cases had panniculitis,3 cases had ganglion cyst;while pathological diagnosis showed that 64 cases had schwannoma and 52 cases had neurofibromatosis.Af-ter that,ultrasonic diagnosis confirmed coincidence rate of 70.9% in schwannoma,65.1% in neurofibromas and 84.5% in neu-rogenic tumors,so misdiagnosis rate was 1 5.5%;The detection rate of surface solitary schwannomas and neurofibromas found no significant difference (χ2 =0.74,Ρ>0.05 ).Conclusions Ultranosound may be applied to differential diagnosis of solitary schwannoma surface and neurofibromas in clinic based on their relationships with the peripheral nerves.
