目的：对2009-2013年仙桃市基本医疗服务的均等化水平进行评价，全面反映仙桃市基本医疗服务乡镇间差异和城乡差异，为推进县域基本医疗卫生服务均等化提供参考依据。方法从“乡镇-城乡总体”两个维度，运用基尼系数、Kakwani 指数分析方法实证分析仙桃市执业(助理)医师数、卫生医疗机构床位数以及政府卫生支出的均等化程度及变化趋势。结果2009—2013年仙桃市乡镇间的基尼系数均下降，执业(助理)医师数、卫生医疗机构床位数的总体基尼系数是上升的，政府卫生支出的总体基尼系数与 Kakwani 指数是下降的。结论2009—2013年仙桃市城乡医疗卫生资源分配差距悬殊且在逐年增大；政府卫生支出城乡差距依然较大，但是呈现出缩小的趋势。建议仙桃市政府优化医疗卫生资源配置，加大对农村地区的卫生财政支出比重，促进均衡发展。%Objectives To analyze the equalization of basic medical and health services and reflect the differences among towns and between urban-rural of Xiantao in Jianghan Plain during the period of 2009 to 2013,which can be used for policy making.Methods To describe and analyze the equity of physicians (assistant),hospital-beds and government health ex-penditure of Xiantao during 5 years with Gini coefficient and Kakwani index.Results From 2009 to 2013,the Gini coef-ficients among 1 5 towns all fall,while the Gini coefficients of physicians (assistant)and hospital-beds among the whole city increase.The Gini coefficients and Kakwani index of government health expenditure among the cities both fall.Conclusions The urban-rural gap of health resource allocation is large and become worse,while the urban-rural gap of government health expenditure is smaller.The government should optimize the allocation of health re-sources and increase the proportion of health expenditure in rural areas to fostering the balanced development.
