转颈试验对椎间隙段椎动脉频谱形态影响的分析

         

摘要

Objective To investigate the influence of neck rotation test on Doppler spectrum of intervertebral artery,and to analyse the abnormal Doppler spectrum,hemodynamic change and the effect on predicting the distal vertebral arterial stenosis.Methods 183 patients with no abnormality in the imaging examination of vertebral artery were recruited.The patients were divided into positive group(n=11)and negative group(n =172) according to whether the abnormal spectrum appeared after neck rotation test.Two-dimensional image and Doppler ultrasound were used to study the diameters and blood flow images of intervertebral artery such as the peak systolic velocity(PSV),the end diastolic velocity(EDV),the resistance index(RI),the acceleration time(AT) and heart rate (HR) in these cases before and after the neck rotation test.Results (1)11 of 183 cases(6.01%)with low systolic velocity and without diastolic velocity of intervertebral artery were observed.Meanwhile,3 of these cases had such spectrum on both sides after the test.(2)No statistically significant differences were found for all parameters between the positive group and negative group before the test.(3) After the test,the PSV,EDV and AT decreased and RI increased largely in the positive group than those before the test and those in the negative group.All the differences were statistically significant.(4) After the test,the Doppler spectrum and the parameters of the negative group did not show a significant difference when compared with those before the test.(5)The overall incidence of vertigo of the positive group was higher than the nagative group,but the difference was not statistically significant.Conclusions The spectrum of low systolic velocity and without diastolic velocity of intervertebral artery may occur with a certain probability value after the neck rotation test.Therefore,when doing ultrasound examination of the cervical arteries,we should pay more attention to keeping the neck in neutral position to avoid misdiagnosis of severe stenosis or occlusion of intracranial vertebral artery due to the change of such spectrum caused by the neck rotation.%目的 探讨转颈试验对椎间隙段椎动脉多普勒频谱的影响,分析出现的异常频谱形态、血流动力学参数改变以及可能对预测远端椎动脉狭窄闭塞性病变的影响.方法 选择椎动脉影像学检查未见异常的183例患者,以转颈试验后是否出现频谱形态异常分为阳性组(11例)和阴性组(172例),超声检测椎间隙段椎动脉管径大小及转颈试验前后血流动力学参数,包括收缩期峰值流速(PSV)、舒张末期流速(EDV)、阻力指数(RI)、加速时间(AT)、心率(HR).结果 (1) 183例患者中,有11例(占6.01%)在转颈试验后即刻出现频谱形态改变,呈“低速单峰”样频谱形态,其中有3例双侧椎动脉频谱转颈试验后均出现异常.(2)转颈试验前阳性组与阴性组比较各血流动力学参数间差异均无统计学意义.(3)转颈试验后阳性组频谱形态异常改变,PSV、EDV、AT均较转颈试验前明显减低,RI明显增加,差异有统计学意义,同时与阴性组比较有显著性差异.(4)转颈试验后阴性组频谱形态无明显改变,各血流动力学参数与转颈试验前比较差异均无统计学意义.(5)“低速单峰”组转颈试验后眩晕发生率较对照组高,但无显著性差异.结论 椎动脉“低速单峰”频谱形态改变在转颈试验时会有一定的概率出现,因此,在做颈动脉超声检查时,应尽量使患者颈部保持中立位,避免因转颈后导致的频谱形态改变而误诊为颅内段椎动脉重度狭窄或闭塞.
