目的 探讨孕妇胎膜早破对分娩结局和新生儿的影响.方法 选取重庆医科大学附属大学城医院2014年8月至2016年8月两年间分娩的足月孕妇胎膜早破病例 200 例,设为观察组,随机抽取同期足月健康孕妇200例,设为对照组,比较两组孕产妇分娩结局.结果 本院在2014年8月至2016年8月两年分娩新生儿2000例,其中间胎膜早破的发生率约为10.00%(200/2 000),新生儿的出生体重在1~5kg之间,平均(3.36±0.44)kg;出生身长在47~55cm之间,平均(50.38±1.05)cm;分娩时胎龄在24.57~41.47周之间,平均(39.24±1.92)周.孕妇分娩方式为观察组自然分娩者103例,占51.50%,剖宫产97例,占48.50%;对照组自然分娩者168例,占84.00%,剖宫产32例,占16.00%,差异有统计学(χ2=29.29,P<0.001).观察组中阴道分娩的产妇第一、第二、总产程时限均显著低于对照组中阴道分娩的产妇(t值分别为2.92、3.69、7.37,均P<0.05),两组第三产程比较无显著性差异(t=0.70,P>0.05).观察组与对照组中产后出血量比较差异无统计学意义(t=0.71,P>0.05);观察组中新生儿Apgar评分1分钟、5分钟均显著低于对照组,差异有统计学意义(t值分别为16.82、8.24,均P<0.05).结论 胎膜早破的发生率较高对分娩结局和新生儿有着较大的影响,临床上应给予重视,提高分娩质量.%Objective To explore the influence of premature rupture of membranes on childbirth outcomes and neonates.Methods Altogether 200 full term pregnant women with premature rupture of membranes giving birth in University Town Hospital Affiliated to Chongqing Medical University from August 2014 to August 2016 were selected as observation group.At same period 200 full term healthy pregnant women were randomly selected as control group.Delivery outcomes were compared between two groups.Results A total of 2 000 neonates were delivered in the hospital from August 2014 to August 2016, and the incidence of premature rupture of membranes was 10.00% (200/2 000).Neonatal birth weight was between 1 and 5kg, with average weight of 3.36 ± 0.44kg.Birth length ranged between 47 to 55cm, with average length of 50.38±1.05 cm.Gestational age at delivery was between 24.57 to 41.47 weeks, with average age of 39.24±1.92 weeks.In observation group there were 103 cases of natural delivery accounting for 51.50% and 97 cases of cesarean section accountinged for 48.50%.In the control group there were 168 cases of natural childbirth, accountinged for 84.00% and 32 cases of cesarean section accounting for 16.00% with significant difference (χ2=29.29, P<0.001).First, second and total stage of labor of pregnant women with vaginal delivery in observation group were significantly lower than those of women with vaginal delivery in control group (t value was 2.92, 3.69 and 7.37, respectively, all P<0.05), but no significant difference was found in third stage of labor between two groups (t=0.70, P>0.05).Postpartum bleeding amount in two groups had no significant difference (t=0.71, P>0.05).In observation group 1min and 5min neonatal Apgar scores were greatly lower than those in control group, and differences were statistically significant (t value was 16.82 and 8.24, respectively, both P<0.05).Conclusion Situation of premature rupture of membranes is serious.It has great effect on delivery outcomes and neonates.So great attention should be paid to it to to improve quality of childbirth.
