目的 研究经直肠超声在青春期残角子宫诊断中的作用.方法 回顾性分析6例经直肠超声诊断为残角子宫的青春期患者,分析其影像学及临床资料,随访其手术及病理诊断.结果 6例经直肠超声诊断为残角子宫的患者中,4例患者(66.7%)进行了手术诊治,其手术前经腹超声或电子计算机X线断层扫描检查均出现误诊,手术后均确诊为残角子宫.经直肠超声诊断青春期残角子宫患者的手术确诊率为66.7%(4/6).经直肠超声技术可增强细微图像的显影能力,增强残角子宫内积血干扰图像的细微显影,卵巢声像图显影清晰,异常声像图与子宫及卵巢声像图的解剖关系清楚.结论 经直肠超声可以明显提高青春期女性残角子宫的诊出率,减少误诊,这对减少残角子宫患者的并发症、及时进行畸形矫正具有重要的临床意义.%Objective To explore the value of transrectal ultrasound in diagnosing rudimentary horn of uterus among adolescents. Methods The imaging and clinical data of 6 adolescent patients diagnosed with rudimentary horn of uterus by transrectal ultrasound was retrospectively analyzed. Their operation and pathological results were followed up. Results Four of 6 cases ( 66. 7% ) were operated who were diagnosed with rudimentary horn of uterus by transrectal ultrasonography. These 4 cases had ever been misdiagnosed by the examination of transabdominal ultrasound and computed tomography ( CT ) before surgery, and they were diagnosed as rudimentary horn of uterus after operation. The final operation diagnosis rate of rudimentary horn of uterus was 66. 7% ( 4/6 ) among patients who had been diagnosed with it by transrectal ultrasonography. The technique of transrectal ultrasonography could enhance the visualization for minute images and could improve minute visualization for haematocele confusing sonogram in rudimentary horn of uterus. The sonogram of ovaries displayed clearly in transrectal ultrasonography. The technique had better acuity for relationship of abnormal sonogram with uterus and ovaries. Conclusion Transrectal ultrasonography can increase diagnose rate and decrease misdiagnosis of rudimentary horn of uterus among adolescent females obviously. It has great clinical significance for reducing complication of rudimentary horn of uterus and correcting deformity in time.
