Objective To assess the overall capability of rescuing critical pregnant women in midwifery medical and health care institutions in Fengtai District of Beijing. Methods A quantitative analysis was conducted to collect the information of administration, basic equipments, technical skills and obstetric quality of all midwifery medical and health care institutions according to The Quarterly Work Quality Report of obstetrics in Beijing Hospital and The Assessment Form of Midwifery Medical Institutions in Beijing ( 2010 ). With qualitative analysis, the rescued cases reported in 2010 were evaluated by the obstetric quality experts group of Beijing and Fengtai District. Results In 2010 there were 11 900 cases of childbearing at 12 midwifery medical and health care institutions in the Fengtai District, and there were 103 critical ones with the occurrence rate of 0. 87% . The perinatal mortality rate of obstetric critical cases was 38. 83 %o, which was significantly higher than the obstetrics average level of Fengtai District ( 5. 29 %c,χ2 = 25. 533 ,P < 0. 001 ). The top three obstetric critical diseases were postpartum hemorrhage, hypertensive disorders in pregnancy and pregnancy complicated with acute fatty liver. Conclusion The capacity of midwifery medical and health care institutions at all levels in rescuing critical pregnant women has reached the standard in Beijing. The incidence of adverse pregnant outcomes of critical pregnant women is relatively high, and medical staff should pay special attention to these patients.%目的 评估北京市丰台区助产医疗保健机构在危重孕产妇抢救方面的整体水平.方法 ①定量分析:利用<北京市医院产科工作质量季报表>和<北京市助产医疗机构验收评估表>(2010年)对全区所有助产医疗、保健机构的行政管理、基本设备、技术技能及产科质量等进行统计;②定性分析:2010年上报危重孕产妇抢救病例,由北京市产科质量专家及丰台区产科质量专家组进行评审.结果 2010年全区12所助产医疗保健机构共有11 900例孕产妇分娩,产科危重症抢救103例,发生率为0.87%.产科危重症病例的围产儿死亡率为38.83‰,明显高于全区产科平均水平5.29‰(χ2=25.533,P<0.001).产科危重症前3位分别为产后出血、妊娠期高血压疾病、妊娠合并急性脂肪肝.结论 丰台区各级助产医疗保健机构的危重孕产妇抢救能力均达到北京市有关标准.危重孕产妇不良妊娠结局发生率较高,医务工作者应特别重视这些患者.
