目的:探讨临床药师在冠心病合并高血压病患者药学监护中的作用.方法:临床药师参与1例冠心病合并高血压病患者的药物治疗,协助临床医师为住院患者制订个体化治疗方案,并实施全程药学监护.结果:在冠心病合并高血压病患者的治疗中,临床药师与临床医师密切合作,调整抗高血压药、抗血栓药等治疗方案,患者病情好转后出院.结论:临床药师开展以患者为中心的药学监护,有利于提高药学服务水平和服务质量,保障患者合理用药.%OBJECTIVE:To explore the role of clinical pharmacists in pharmaceutical care for patient with coronary heart disease complicated with hypertension .METHODS: Clinical pharmacists participated into the drug therapy for one patient with coronary heart disease complicated with hypertension , developed individualized pharmaceutical care plans and carried out the whole course of pharmaceutical care .RESULTS:During the treatment of coronary heart disease and hypertension , the clinical pharmacists closely cooperated with clinicians , adjusted the treatment regimen of antihypertention drugs and antithrombotic drugs , the patients discharged after getting better . CONCLUSIONS:The clinical pharmacists carry out pharmaceutical care for patient as the center can help to improve the pharmaceutical service level and service quality , ensure the rational drug use for patients .
