Objective:To probe into the irrational drug use in medical orders from Pharmacy Intravenous Admixture Services(PIVAS)in Hunan Mawangdui People's Hospital(hereinafter referred to as "our hospital"),so as to promote the rational drug use in clinic.METHODS: Based on instructions,review was conducted on 280 thousands medical orders from PIVAS,and irrational drug use was analyzed.RESULTS: Of the 280 thousands medical orders,3 279 were irrational with the rate of 1.17％,the irrational drug use mainly performed as improper selection of solvent(1 746 medical orders),improper concentration of drugs(625 medical orders),over the highest dosage(124 medical orders),incorrect administration route(109 medical orders)and incompatibility(338 medical orders),etc.The clinical pharmacists intervened the irrational drug use in medical orders.CONCLUSIONS: Centralized review and intervention on medical orders from PIVAS by clinical pharmacists can effectively improve the rationality of intravenous drug use.%目的:探讨湖南省人民医院马王堆院区(以下简称"我院")静脉用药集中调配中心发现的医嘱不合理用药情况,促进临床合理用药.方法:以药品说明书等参考文献为依据,对我院2016年28万条静脉用药集中调配中心的医嘱进行审核,分析医嘱不合理用药情况.结果:28万条静脉用药医嘱中,不合理用药医嘱3 279条,不合理用药率为1.17％,不合理用药主要集中在溶剂选择不当(1 746条)、药物配制浓度不当(625条)、用量超过最高剂量(124条)、给药途径错误(109条)、存在配伍禁忌(338条)等方面.临床药师对不合理用药医嘱进行了干预.结论:药师对静脉用药集中调配中心的医嘱进行集中审核及干预,可以有效提高静脉用药的合理性.
