目的:系统评价在常规抗心绞痛药治疗基础上联合应用辛伐他汀与氯吡格雷对患者白细胞介素6(IL-6)、超敏C反应蛋白(hs-CRP)水平及预后的影响.方法:计算机检索PubMed、Cochrane library、EBSCO及万方医学网、中国知网、维普中文数据库等电子文献库中辛伐他汀与氯吡格雷联合应用于不稳定性心绞痛患者的临床随机对照试验,检索时限为建库至2016年10月.2名研究者根据纳入与排除标准独立筛选文献,并对其进行质量评价.采用RevMan 5.0软件对各效应指标进行文献荟萃(Meta)分析.结果:共纳入7篇文献,涉及629例患者,其中观察组(在常规抗心绞痛药治疗基础上联合应用辛伐他汀与氯吡格雷)315例,对照组(在常规抗心绞痛药治疗基础上加用辛伐他汀)314例.Meta分析结果显示,观察组患者的显效率(OR合并=1.84,95％CI=1.30～2.62,P0.05).结论:在常规抗心绞痛药治疗基础上联合应用辛伐他汀与氯吡格雷治疗不稳定性心绞痛的效果显著,对IL-6、hs-CRP水平有良好的控制效果,安全性高.%OBJECTIVE:To systematically review the effect of simvastatin combined with clopidogrel in basis of conventional therapy of anti-anginal drugs on IL-6 and hs-CRP levels and its prognosis . METHODS: Clinical randomized controlled trials of simvastatin combined with clopidogrel in treatment of patients with unstable angina were retrieved from PubMed, Cochrane Library, EBSCO, Wanfang Database, CNKI, VIP Database, etc.Time period of retrieval was from database established to Oct .2016.2 researchers conducted literature screening independently according to the inclusion and exclusion criteria , and then processed quality evaluation .RevMan5.0 was adopted to conduct Meta analysis on all effective indicators .RESULTS: A total of 7 studies were involved , including 629 patients, of 315 cases were observation group (simvastatin combined with clopidogrel in basis of conventional therapy of anti-anginal drugs ) and other 314 cases were control group ( simvastatin combined with conventional therapy of anti-anginal drugs).Results of Meta analysis showed the marked efficiency rate (ORemerged =1.84, 95％ CI=1.30-2.62, P0.05 ) . CONCLUSIONS:The efficacy of simvastatin combined with clopidogrel in basis of conventional therapy of anti -anginal drugs in treatment of unstable angina is remarkable , which can effectively control the IL-6 and hs-CRP levels, with high safety .
