OBJECTIVE:To evaluate the efficacy of rhGM-CSF plus topical mupirocin ointment for large residual burn wounds as well as its effect on vascular endothelial growth factor.METHODS:67 patients with extensive burns (127 wounds) were randomly assigend to either observation group (35 cases/69 wounds) treated with rhGM-CSF plus topical mupirocin ointment or control group(32 cases/58 wounds) treated with topical mupirocin ointment alone.The response rate, wound healing rate, average healing time, wound bacterial clearance rate and mean optical density value of vascular endothelial growth factor in the two groups were compared.RESULTS: The total response rate and the wound healing rate in the observation group were significantly higher than in the control gorup [92.75%(64/69) and 91.30%(63/69) vs.75.86%(44/58) and 77.59%(45/58)];the mean wound healing time was significantly shorter in the observation group than in the control group [(18.07 ±4.26) days vs.(23.18 ±3.72) days]; the wound bacterial clearance rate in the observation group was significantly higher than in the cotnrol group [ ( 84.62%(22/26) vs.40.74%(11/27);the mean optical density value of vascular endothelial growth factor in the observation group was significantly higher than in the cotnrol group(0.23 ±0.03 vs.0.17 ±0.02), and all the differences were statistically significant ( P <0.05 ) .CONCLUSIONS: The additional of topical rhGM-CSF to mupirocin ointment resulted in remarkable efficacy for residual burn wounds in patients with extensive burn and significantly elevated expression of vascular endothelial growth factor, which thus is of great clinical research value.%目的：探讨外用重组人粒细胞巨噬细胞集落刺激因子（ rhGM-CSF）联合莫匹罗软膏对大面积烧伤残余创面的疗效及其对血管内皮细胞生长因子的影响。方法：采用随机数字表法，将67例大面积烧伤患者（127个创面）分为观察组35例（69个创面）和对照组32例（58个创面）。观察组采用外用rhGM-CSF联合莫匹罗软膏治疗，对照组单用莫匹罗软膏治疗。对比分析2组患者的创面疗效、创面愈合率、创面平均愈合时间、创面细菌清除率及血管内皮细胞生长因子平均光密度值。结果：观察组患者总有效率（92.75％，64／69）显著高于对照组（75.86％，44／58），创面愈合率（91.30％，63／69）显著高于对照组（77.59％，45／58），创面平均愈合时间[（18.07±4.26） d]显著短于对照组[（23.18±3.72） d]，创面细菌清除率（84.62％，22／26）显著高于对照组（40.74％，11／27），治疗2周后血管内皮细胞生长因子平均光密度值（0.23±0.03）显著高于对照组（0.17±0.02），差异均有统计学意义（ P＜0.05）。结论：外用rhGM-CSF联合莫匹罗软膏对大面积烧伤残余创面的效果显著，可明显提高血管内细胞生长因子的表达，具有重要临床研究价值。
