目的：比较卡维地洛与美托洛尔治疗扩张型心肌病合并心力衰竭的临床效果。方法：选择文山州人民医院2012年7月—2014年12月收治的116例扩张型心肌病合并心力衰竭患者，按随机数字表法分为观察组和对照组各58例，在内科常规治疗的基础上，对照组患者采用美托洛尔治疗，观察组患者采用卡维地洛进行治疗，比较2组临床效果及治疗前后的左室射血分数、左室舒张末内径和6 min步行距离。结果：观察组患者总有效率为87.93％（51／58），明显高于对照组的74.14％（43／58），差异有统计学意义（P＜0.05）；2组患者左室射血分数、左室舒张末内径和6 min步行距离均明显优于治疗前，且观察组患者各指标改善程度优于对照组，差异均有统计学意义（ P＜0.05）。结论：与美托洛尔比较，卡维地洛治疗扩张型心肌病合并心力衰竭的临床效果更加显著，可有效的改善患者的心功能，值得临床推广。%OBJECTIVE:To probe into the clinical efficacy of carvedilol and metoprolol in treatment of dilated cardiomyopathy with heart failure .METHODS: 116 heart failure patients with dilated cardiomyopathy admitted in Wenshan People's Hospital from Jul.2012 to Dec.2014 were divided into observation group and control group via random number table , with 58 cases in each group .The control group were treated with metoprolol based on conventional medical treatment , and the observation group received metoprolol .The clinical efficacy and the left ventricular ejection fraction ( LVEF), left ventricular end diastolic diameter ( LVDD) and 6min walking distance before and after treatment was compared in two groups .RESULTS: The total effective rate of observation group was 87.93%( 51/58 ) , significantly higher than that of the control group [ 74.14%( 43/58 ) ] , the difference was statistically significant ( P <0.05 ).After treatment, LVDD, LVEF and 6min walk distance in two groups were significantly better than those of before treatment , and the improved indexes in observation group were better than that of control group, with statistically significant difference (P<0.05).CONCLUSIONS:Compared with metoprolol, the clinical efficacy of carvedilol in treatment of dilated cardiomyopathy with heart failure is more significant , can effectively improve the patient's cardiac function , and it is worthy of clinical popularization and application .
