目的：探讨临床药师在抗凝治疗中的作用。方法：回顾性分析临床药师参与1例肺动脉栓塞患者华法林抗凝治疗过程。结果：临床药师参与了华法林抗凝治疗方案的制定和调整，分析了华法林抗凝过量的原因，对患者实施了药学监护。结论：临床药师在华法林抗凝治疗中发挥着重要作用，可降低患者抗凝治疗风险，提高患者抗凝治疗成功率。%OBJECTIVE: To explore the role of clinic pharmacist in the anticoagulant treatment.METHODS:Clinic pharmacist ’ experience with the treatment of one patient with pulmonary embolism receiving warfarin anticoagulant therapy was analyzed retrospectively.RESULTS:Clinical pharmacist participated in the formulation and adjustment of warfarin anticoagulant therapy, analyzed the reason for overdose of warfarin as anticoagulant drug and provided pharmaceutical care for patients.CONCLUSIONS: Clinical pharmacists play an important role in warfarin anticoagulant therapy, and their involvement can help reduce the risk of anticoagulant therapy and improve the success rate of anticoagulant therapy.
