目的:通过定量分析畸变产物耳声发射(DPOAE)幅值下降的程度与单侧突发性耳聋预后的相关性,为单侧突发性耳聋预后提供一项较为客观的评价指标。方法选取单侧突发性耳聋患者,治疗前行双耳DPOAE检测,以健耳DPOAE的幅值作为患耳发病前DPOAE幅值的参照,以患耳幅值下降的面积所占的比例判定患耳毛细胞的损伤程度。对治疗前患耳DPOAE幅值下降程度与治疗后有效率行统计学分析。结果将患耳治疗前DPOAE幅值下降程度分为轻,中,重及极重四个等级,治疗后各等级间有效率经统计分析差异,随着幅值下降程度加重,有效率明显降低。结论定量分析DPOAE幅值下降的程度可初步评价单侧突发性耳聋的预后,为单侧突发性耳聋患者预后评估提供一项较为客观的指标,同时为DPOAE在临床中的应用提供新的思路。%Objective To explore the relationship between distortion product otoacoustic emission(DPOAE) reduction and prognosis of unilateral sudden hearing loss(SHL) and find an objective indicator for prognosis evaluation of unilateral SHL. Methods A total of 76 patients with unilateral SHL were enrolled in this study. DPOAE of bilateral ears was detected before the treatment. The normal DPOAE level of the ear with SHL was evaluated according to the DPOAE level of healthy ear in the same patient. The DPOAE reduction was calculated to access the damage degree of the outer hair cell. The relationship between DPOAE reduction and curative effect after treatment was subject toχ2 analysis. Results The DPOAE reduction was divided into 4 degree(mild, moderate, severe and extremely severe). The efficiency rates in this four degree were significant different with each other. The high degree DPOAE reduction means low efficiency rate. Conclusions The quantitative analysis of DPOAE reduction is helpful for the prognosis of unilateral SHL as an objective index.
