目的 研究经皮穴位电子灸三阴交、合谷、足三里联合硬膜外自控镇痛(PCEA)对产程进展的影响.方法 采用回顾性分析的方法,将2015年1月至2016年1月100例足月初产妇随机分为对照组和试验组,每组50例.对照组给予硬膜外自控镇痛,试验组在对照组的基础上,联合应用经皮穴位电子灸,比较2组患者分娩时第一产程、第二产程及第三产程的时间,产后出血量,镇痛效果评价,新生儿 Apgar 评分、产妇下肢运动神经阻滞评分(Bromage 评分)等情况.结果 2组初产妇的年龄、身高、体重、孕周等各项指标差异无统计学意义(P>0.05),具有可比性.试验组产妇第一产程时间、产后出血量明显低于对照组,差异有统计学意义(P<0.05);试验组产妇下肢运动神经阻滞评分高于对照组,差异有统计学意义(P<0.05);第二产程和第三产程的时间以及新生儿Apgar评分差异无统计学意义(P>0.05).结论 经皮穴位电子灸三阴交、合谷、足三里穴位联合硬膜外自控镇痛对产妇的镇痛效果显著,第一产程时间明显缩短,产后出血量明显减少、下肢运动神经阻滞情况有所减轻,此方法安全有效,值得临床推广.%Objective To investigate the effects of percutaneous acupoints electronic moxibustion including the acupoints of Hegu,Sanyin jiao and Zusanli combined with patient controlled epidural analgesia (PCEA) on progress of birth process.Methods One hundred mature primipara who were treated in our hospital were divided into two control group and experiment group,with 50 primipara in each group.The primipara in control group were treated by patient controlled epidural analgesia,however,the primipara in experiment group, on the basis of control group,were treated by percutaneous acupoints electronic moxibustion. The duration of three birth process,postpartum bleeding volume,the analgesic effects,neonatal Apgar score,Bromage score were observed and compared between two groups. Results There were no significant differences in primipara's age,height,body weight and gestational weeks between the two groups(P>0.05). The duration of first stage of labor,postpartum bleeding volume in experiment group were significantly lower than those in control group (P <0.05), however,the Bromage scores in experiment group were significantly higher than those in control group(P<0.05),In addition there were no significant differences in the duration of second stage of labor and third stage of labor as well as Apgar scores between two groups (P>0.05).Conclusion The percutaneous acupoints electronic moxibustion including the acupoints of Hegu,Sanyin jiao and Zusanli combined with patient controlled epidural analgesia has obvious analgesia effeects,which can shorten the duration of first stage of labor,decrease postpartum bleeding volume,relieve lower limb motor nerve block,with sefaty and effectiveness,thus,which is worth using widely in clinical practice.
