肾脏肿瘤患者CT灌注参数与肾功能生化指标和肿瘤标志物的相关性研究

         

摘要

目的 探讨肾脏肿瘤患者CT灌注参数与肾功能生化检测指标和肿瘤标志物的相关性.方法 选取2012年3月至2017年2月收治的36例肾脏肿瘤患者为观察组,另选取同期接受健康体检的34例健康体检者为对照组,均对2组实施CT灌注成像,获取等效血容量(Equiv BV)、表面渗透性(Ps)、血流量(BF)等CT灌注参数,并检测2组尿素氮(BUN)、肌酐(Scr)、胆固醇(TC)、三酰甘油(TG)等肾功能生化指标及肿瘤标志物癌胚抗原(CEA)、糖类抗原125(CA125)、细胞角蛋白19片段(Cyfra21-1),比较2组研究对象上述指标检测结果,并对CT灌注参数与肾功能生化指标及肿瘤标志物的相关性进行分析.结果 观察组Equiv BV、Ps、BF均低于对照组(P<0.05),BUN、Scr、TC、TG、CEA、CA125、Cyfra21-1均高于对照组(P<0.05).Pearson相关性分析结果显示观察组CT灌注参数与肾功能生化指标BUN、TC及肿瘤标志物CEA、CA125、Cyfra21-1均呈负相关(P<0.05),与Scr、TG无显著相关性(P>0.05).结论 肾脏肿瘤患者的CT灌注参数与肾功能生化检测指标及肿瘤标志物的表达呈负相关性,可作为评估肾脏肿瘤患者肾功能及疾病临床诊断的辅助方法.%Objective To investigate the correlation between CT perfusion parameters and the biochemical detection indexes of renal function as well as tumor markers in patients with renal neoplasms. Methods Thirty-six patients with renal tumors who were admitted and treated in our hospital from March 2012 to February 2017 were enrolled as observation group,at the same time, the other 34 healthy subjects who received physical examination in our hospital were enrolled as control group. The equivalent blood volume ( Equiv BV) , surface permeability ( Ps) , blood flow ( BF) and other CT perfusion parameters were abtained by CT perfusion imaging in both groups. Moreover the biochemical detection indexes including blood urea nitrogen ( BUN) , serum creatinine ( Scr) , total cholesterol ( TC) , triglyceride ( TG) and tumor markers including carcino-embryonic antigen (CEA), carbohydrate antigen 125 (CA125), and cytokeratin 19 fragment (Cyfra211) were detected and compared between two groups, and the correlation between CT perfusion parameters and the biochemical indexes of renal function as well as tumor markers was analyzed by means of Pearson analysis. Results The levels of Equiv BV, Ps and BF in observation group were significantly lower than those in control group (P<0. 05), however the levels of BUN, Scr, TC, TG, CEA, CA125 and Cyfra211 in observation group were significantly higher than those in control group (P <0. 05). The Pearson correlation analysis showed that CT perfusion parameters were negatively correlated with the biochemical markers of renal function including BUN, TC and tumor markers including CEA, CA125 and Cyfra211(P<0. 05), however, there was no close correlation between CT perfusion parameters and the levels of Scr and TG (P >0. 05). Conclusion The CT perfusion parameters of patients with renal tumor are negatively correlated with the biochemical indexes of renal function and expressions of tumor markers,which can be regarded as an auxiliary method to evaluate the renal function of patients with renal tumor and clinical diagnosis of disease.
