Objective To analyze the clinical effect of neoadjuvant chemotherapy in the treatment of advanced gastric cancer and to detect its influence on the expressions of vascular endothelial growth factor(VEGF) and matrix metalloproteinase-2 (MMP-2).Methods 70 patients with advanced gastric cancer were randomly divided into an experimental group(n=35)and a control group(n=35).The control group were given surgery treatment and general drug chemotherapy,while in addition,the experimental group were given neoadjuvant chemotherapy (S-1 combined with oxaliplatin).The incidence of cancer recurrence and the expressions of MMP-2 and VEGF of the two groups were observed.Results In the experimental group,the expressions of MMP-2 or VEGF were significantly lower after than before the chemotherapy(P＜0.01).The positive rate of lymph nodes was significantly lower in the experimental group than in the control group (9.24 ％ vs.18.89 ％),with a statistical difference (P＜0.01);what's more,the 3-years survival rate was significantly higher in the experimental group than in the control group (71.43％ vs.45.71％),with a statistical difference (P＜0.05).Conclusions Neoadjuvant chemotherapy could reduce the expressions of MMP-2 and VEGF in tumor tissues,indicating that it may play an important role in patients with advanced gastric cancer and can be used for clinics.%目的 探讨新辅助化疗(奥沙利铂联合替吉奥)对进展期胃癌的治疗效果,并研究术前应用新辅助化疗对胃癌组织中血管内皮生长因子(VEGF)及基质金属蛋白酶2(MMP-2)表达的影响.方法 选取进展期胃癌患者70例,并随机分为对照组及实验组各35例,对照组采用胃癌根治术联合术后常规化疗的治疗方案,实验组患者在对照组治疗的基础上行术前新辅助化疗.分析两组患者术中淋巴结检查阳性率、术后肿瘤复发率、患者3年存活率及化疗前后癌组织中MMP-2和VEGF表达的变化.结果 胃癌患者术前应用新辅助化疗可以有效降低胃癌组织MMP-2和VEGF的表达(P＜0.01).实验组胃癌患者术中检出淋巴结阳性率(9.24％)明显低于对照组患者淋巴结阳性率(18.89％),差异有统计学意义(P＜0.01);实验组胃癌患者3年内存活率(71.43％)显著高于对照组患者存活率(45.71％),两组比较差异具有统计学意义(P＜0.05).结论 胃癌患者术前应用奥沙利铂联合替吉奥新辅助化疗方案能够显著提高胃癌患者的临床受益率,降低胃癌组织中MMP-2和VEGF的表达,提示胃癌患者术前应用新辅助化疗具有重要的临床意义,值得推广.
