前牙残根根管充填后即刻桩道预备的临床观察

摘要

目的 探讨前牙残根热牙胶根管充填后即刻桩道预备的临床效果.方法将96颗应用热牙胶根管充填的前牙残根随机分为试验组和对照组.试验组48颗残根即刻桩道预备,对照组48颗残根观察一周后再行桩道预备.两组均采用硅橡胶间接印模法取模,铸造桩核,最后完成烤瓷冠修复.评价两组桩冠修复的临床效果,并行统计学分析.结果 术后24 h试验组3颗疼痛发作,对照组1颗疼痛发作.两组24 h内疼痛率相比较,没有统计学差异(P>0.05).第一年试验组2颗轻微咀嚼不适感,2颗桩冠修复体松动脱落;对照组2颗轻微咀嚼不适感,1颗桩冠修复体松动脱落.第二年试验组4颗咀嚼不适,牙周膜间隙增宽,2颗桩冠修复体松动脱落;对照组2颗咀嚼不适,牙周膜间隙增宽,3颗桩冠修复体松动脱落.结论 热牙胶充填前牙残根根骨后即刻桩道预备、桩冠修复临床效果良好,可以缩短患者的修复时间.%Objective To explore efficacy of immediate post space preparation on anterior residual root canals obturated with warm vertical condensation of gutta-percha. Methods A total of 96 anterior residual roots were obturated with warm vertical condensation of gutta-percha. The teeth were randomly divided into two groups. The post spaces of 48 roots were prepared immediately after filling as study group, while the other 48 roots were prepared after a week as control. Two groups were impressed by using of silicone rubber to make the casting model. The efficacy of the procedures for post crowns was compared between the two groups. Results 3 residual roots in the study group and one in the control group occurred ache 24 hours after preparation, with no significant difference between the two groups ( P >0.05 ). In the first year, 2 teeth masticated discomfortably and 2 shed their post crowns in the study group while 2 teeth masticated discomfortably and one tooth shed its post crown in the control group. In the second year, 4 teeth masticated discomfortably and 2 shed their post crowns in the group whereas 2 masticated discomfortably and 3 shed their post crowns. Conclusions Immediate post space preparation on anterior residual root canals obturated with warm vertical condensation of gutta-percha can shorten the course of treatment for patients and yield good outcomes.
