正常新生儿多频声导抗特征分析

         

摘要

Objective To analyze the clinical characteristic of multifrequency- tympanometry obtained from normal newborns who passed the transiently evoked otoacoustic emission (TEOAE) screening. Methods TEOAE screening was performed with GSI-70 automated OAE. The multifrequency tympanograms were obtained from 490 newborns who passed the TEOAE screening in both ears and were on the normal physiological condition after born with GSI Tympstar Version II Middle Ear Analyzer. To collect the probe 226 Hz, 678 Hz,l 000 Hz tympanograms, including admittance (Y), susceptance (B), conductance (G); then they were tested by multiple frequency tympanometry. The probe- tone frequencies ranged from 250 to 2 000 Hz. Results To group the multifrequency tympanograms by the Vanhuyse model. There were 1B1G type,3BlG type,3B3G type,5B3G type and other types, 3B3G type dominated in probe 226 Hz, about 61. 02%; 1B1G type dominated in probe 678 Hz, about 58. 37%; 1B1G type dominated in probe 1 000 Hz, about 36. 43%. There was a statistical significance in the three frequency proportionate(P<0. 05). The 95% confidence interval of the tympanometric datas were as follows; the resonance frequency was about 182.18~346. 04 Hz,the compensated static admittance (△B) was about -0. 65~0.05 mmho, the compensated static phase angle (△θ) was about ~83. 23°~18. 05°. Conclusion The resonance frequency range of middle ear is 182.18~346. 04 Hz in newborns. The research addends the datas of multifrequency-tympanometry in normal neonates, that may serve as a guide for newborn hearing screening and detecting middle ear function in newborns.%目的 分析正常听力新生儿多频声导抗测试的特点,为新生儿听力筛查及新生儿中耳功能的评估提供参考依据.方法 按照正常新生儿纳入标准选人490例(980耳)正常新生儿,采用GSI TympStar VersionⅡ中耳分析仪对其进行中耳功能测试,分析226、678、1000 Hz声导抗的声导纳Y(admittance)、声纳B(susceptance)、声导G (conductance)及250~2000Hz多频率扫描鼓室导抗图.运用SPSS13.0统计学软件对各指标的结果进行统计学处理,计算均数、标准差以及95%可信区间.结果 将980耳的鼓室导抗图按Vanhuyse模型分类,分为1B1G型、3B1G型、3B3G型、5B3G型及其它型,226 Hz鼓室导抗图以3B3G型占主导,约为61.02%;678及1000Hz鼓室导抗图以1B1G型占主导,分别约为58.37%和36.43%,三个频率之间鼓室导抗图分型比例比较差异有统计学意义(P<0.05).正常听力新生儿的中耳共振频率、补偿静态声纳值(compensated static admittance,△B)、补偿静态相位角(compensated static phase angle,△θ)的95%可信区间分别为:182.18~346.04、-0.65~0.05mmho、-83.23~18.05°.结论 新生儿的中耳可能以质量控制为主;正常新生儿226 Hz声纳图B多以双峰型为主.
