目的 分析退变性腰椎管狭窄减压术后影响下肢疼痛缓解的因素.方法 将156例退变性腰椎管狭窄下肢疼痛患者采用不同的减压术进行治疗,分析年龄、病程、体质指数、术前腰椎稳定性、椎管多节段狭窄、健康状态分级、椎管减压术式对术后下肢疼痛缓解的影响.结果 单因素分析结果显示,年龄、病程、术前腰椎不稳、椎管多节段狭窄、健康状态分级均与疗效存在相关性,且差异均有统计学意义(P＜0.05). Logistic多元回归分析发现,术前腰椎不稳、椎管多节段狭窄、高健康状态分级为影响椎管减压术后下肢疼痛缓解的独立危险因素(P＜0.05).结论 术前腰椎不稳、椎管多节段狭窄、高健康状态分级可作为评估退变性腰椎管狭窄减压术指针和疗效评价指标.%Objective To analyze the relevant factors of melosalgia alleviation after surgical decompression for degen-erative lumbar spinal stenosis. Methods Retrospective analysis was performed to research 156 patients from accord-ing with the inclusion and exclusion criteria. The age,course of disease body mass index, preoperative stability of lumbar spine, segments involved, health class and surgical procedures were selected for possible relevant factors. Or-dinal logistic regression analysis was performed to determine relevant factors. Results According to univariate analy-sis, age, course of disease, preoperative instability of lumbar spine, segments involved, health class were associated with adverse health effect(P＜0.05). Preoperative instability of lumbar spine, segments involved, high health class were risk factors of no avail through logistic regression analysis(P＜0.05). Conclusions Preoperative instability of lumbar spine, segments involved, health class can be used as an indicator of decompression and evaluation of degen-erative lumbar spinal stenosis.
