Objective To explore treatment effect and strategy of postoperative early wound deep infection in spinal surgery with instrumentation.Methods The 9 cases with postoperative early wound deep infection in spinal surgery with instrumentation were treated by debridement,closed suction irrigation and vacuum sealing drainage(VSD).Results Number of surgical debridement was 1 ～ 8 times,all cases were cured,3 cases with complete retention of spinal instrumentation and 3 cases with partial retention,3 cases were replaced pedicle screw-rod system.Duration of hospitalization was 28 ～150 d.The follow-up time was 12 ～24 months.During the follow-up period,no recurrence of infection except 1 case was removed instrumentation after successful intervertebral fusion,because recurrence of infection occurred at 1 year postoperation.Condusions Most cases of postoperative early wound deep infection in spinal surgery with instrumentation can be cured with retention of instrumentation by aggressive debridement,closed suction irrigation and VSD combined with sensitive antibiotics.%目的 探讨脊柱内固定术后早期切口深部感染的治疗策略及疗效.方法 采用清创、置管冲洗与负压封闭引流治疗9例脊柱内固定术后早期切口深部感染患者.结果 9例患者均获治愈,清创次数为1～8次.3例内固定装置完整保留,3例更换内固定系统,3例内固定部分保留.住院时间28 ～ 150 d.患者均获得随访,时间12 ～24个月.1例术后1年感染复发取出内固定后治愈,其余患者均未复发.结论 脊柱内固定术后早期切口深部感染,通过清创、置管冲洗或负压封闭引流加敏感抗生素治疗,大部分可在保留内固定的同时治愈感染.
