miR-17-5p与乳腺癌侵袭及预后的相关性研究

         

摘要

目的 探讨miR-17-5p在乳腺癌中的表达及其与乳腺癌侵袭、转移及预后的关系.方法 对88例乳腺癌原发肿瘤和20例癌旁乳腺组织的石蜡组织块重新切片,提取RNA,采用实时定量PCR方法 测定miR-17-5p表达水平;Mann Whitney-U非参数检验分析miR-17-5p表达水平与乳腺癌临床病理参数的关系,预后分析采用Kaplan-Meier生存分析法和Log-rank检验.结果 在人表皮生长因子受体-2(HER-2)过表达或伴有脉管侵犯的乳腺癌中,miR-17-5p表达水平明显低于HER-2阴性、无脉管侵犯的乳腺癌(P<0.05或P<0.001);miR-17-5p表达与乳腺癌肿瘤大小、淋巴结转移、组织学分级、雌激素受体(ER)表达、孕激素受体(PR)表达、组织学类型、肿瘤病理分期、是否复发及患者月经状态等无明显相关(P>0.05).以平均值2-△Ct=0.836 作为截断值,Kaplan-Meier生存分析显示,miR-17-5p表达水平与乳腺癌无复发生存率或总生存率无显著相关(P>0.05).结论 miR-17-5p表达与乳腺癌HER-2基因表达及脉管侵犯呈负相关,可做为乳腺癌侵袭潜能的预测指标;miR-17-5p表达水平不影响乳腺癌的预后,其作用机制值得进一步研究阐明.%Objective To investigate the relationship between expression of miR-17-5p and invasiveness or prognosis of breast cancer. Methods Extracting miRNA from the formalin-fixation and paraffin-embedding tissue samples derived from 88 breast cancer patients who underwent mastectomy, miR-17-5p levels were quantified by real-time quantitative polymerase chain reaction (RT-qPCR). The miR-17-5p levels among clinicopathological variables were accessed by Mann Whitney-U test. Recurrence-free survival(RFS) and overall survival(OS) curves were derived from Kaplan-Meier estimates and the curves were compared by Log-rank tests. All statistical tests were two-sided. Results A potential relationship between miR-17-5p levels and existing clinicopathological parameters of breast cancer, such as menstrual status, tumor size, nodal involvement, stage of disease, hormone receptor status, histological grade, tumor subtype or tumor relapse was investigated. No statistically significant difference were identified for these features(P>0. 05). The miR-17-5p level was significant lower among HER-2 negative group than that in HER2 over-expression group(P<0. 05). Lower miR-17-5p level was found in breast cancer with tumor vessel invasion compared to those absences of vessel invasion (P<0. 001). When considering 2-△Ct=0. 836 (mean level) as a cut-off value, no correlation with prognostic outcomes were found according to miR-17-5p expression (high vs low, P>0. 05). Conclusions We suggested that miR-17-5p level is associated with HER2 status and vessel invasion, referring to invasiveness of breast cancer. The miR-17-5p level was found to be not correlated with prognosis in breast cancer, which warrants investigation to further elucidate.
