Objective:To observe the change of QT interval before and after successful cardiopulmonary resuscitation (CPR) in a rabbit model of ventricular fibrillation (VF).Methods:A rabbit model of VF was established and then began CPR.After successful resuscitation,the rabbits were divided into a malignant arrhythmia (MA) group and a non-MA group.The electrocardiogram was observed before VF and after successful restoration of spontaneous circulation (ROSC).Results:The survival rate was significantly lower in the MA group than that in non-MA group (P＜0.05).After successful ROSC,the QTc (corrected QT) and QTpc (corrected QTp) were notably shortened in the MA group (P＜0.05).At 15 min after ROSC,the QTc and QTpc were returned to the normal levels.However,there were no significant differences in QTc and QTpc in non-MA group before VF and after ROSC (P＞0.05).The QTpc in the MA group was significantly shorter than that in non-MA group after successful ROSC (P＜0.05).In the MA group,The corrected T peak-T end at the time of successful return of ROSC and 15 min after ROSC were significantly prolonged as compared with those before VF (P＜0.05).But no significant difference was found in non-MA group (P ＞0.05).Conclusion:The change of QT interval at early ROSC could predict occurrence of MA.Corrected T peak-T end was more sensitive than QTc and QTpc.%目的:观察室颤家兔心肺复苏前、后QT间期变化,探讨其与复苏后恶性心律失常(MA)的关系.方法:制备家兔心室颤动(VF)模型,于心肺复苏(CPR)成功后根据其心电图(ECG)变化分为MA组和非MA组.比较两组在诱发室颤前和自主循环恢复(ROSC)后不同时间点QT间期、QT波峰间期(QTp)、T波顶峰后宽度(T peak-T end间期)校正值的差异.结果:MA组家兔ROSC后存活率明显低于非MA组(P＜0.05).MA组在ROSC即刻心率校正后的QT间期(QTc)和心率校正的QTp值(QTpc)与诱发室颤前比较明显缩短(P＜0.05),ROSC后15 min时,QTc和QTpc已经恢复至诱发室颤前水平;非MA组在ROSC后各时间点的QTc和QTpc与诱发室颤前比较差异均无统计学意义(P＞0.05),MA组ROSC即刻的QTpc明显短于非MA组(P＜0.05);MA组在ROSC即刻和ROSC后15 min时的T peak-T end校正值与诱发室颤前比较明显延长(P＜0.05),但非MA组在ROSC后各时点的T peak-T end校正值与诱发室颤前比较差异均无统计学意义(P＞0.05).结论:ROSC早期QT间期的变化可作为预测MA的重要指标,其中复苏早期T peak-T end校正值延长的敏感性可能比QTc及QTpc更高.
