Objective:To investigate the relationship between different types of epilepsy and the onset time after stroke,and observe the characteristics of video EEG in various types of patients.Methods:A total of 82 patients with post-stroke epilepsy admitted in our hospital from February 2015 to February 2021 were enrolled in this study.The types of stroke,type of epilepsy and the onset of epileptic seizures were recorded.The video EEG features in various types of patients were compared.Results:The incidence of early onset epilepsy after ischemic stroke was 37.21％,which was lower than that in patients with hemorrhagic stroke (84.62％),and the incidence of delayed epilepsy was higher than that of hemorrhagic stroke (P＜ 0.05).The incidence of delayed epilepsy was 73.33％,which was higher than that of partial secondary seizures (12.50％) and systemic seizures (20.69％) (P＜0.05).The EEG showed that 42 cases (51.22％) had epilepsy-like discharges,23 cases (28.05％) had localized slow wave activity,14 cases (17.07％) had asymmetric amplitude on both sides,and 9 cases (10.98％) had partial lead voltage change.Conclusion:There were differences in epilepsy onset time of patients with different epilepsy types after stroke.Partial seizures were mainly tardive epilepsy,and generalized seizures were early-onset epilepsy.Clinical epileptic seizures could be evaluated by observing EEG characteristics.%目的:探讨脑卒中后不同癫痫类型与其发作时间的关系,并观察各类型患者的视频脑电图特征.方法:选取2015年2月至2017年2月陕西省延安大学咸阳医院收治的脑卒中后癫痫患者82例,记录其脑卒中类型、癫痫类型及癫痫发作开始时间,分析其相关性,记录并比较各类型患者视频脑电图特征.结果:缺血性卒中后早发性癫痫发生率为37.21％,低于出血性卒中患者的84.62％,而迟发性癫痫发生率高于出血性卒中(P＜0.05);部分性发作患者的迟发性癫痫发生率为73.33％,较部分性继发全身性发作(12.50％)、全身性发作(20.69％)患者更高(P.＜0.05);经脑电图仪监测发现,有42例(51.22％)为癫痫样放电,23例(28.05％)可见局限性慢波活动,14例(17.07％)两侧波幅明显不对称,9例(10.98％)部分导联电压改变.结论:脑卒中后不同癫痫类型患者的癫痫发生时间存在差异,部分性发作主要为迟发性癫痫,全身性发作则为早发性癫痫,临床可通过观察脑电图特点评估癫痫发作情况.
