目的 探讨经导管溶栓治疗急性下肢缺血(ALI)的方法与疗效.方法 16例ALI患者,按Rutherford分期,Ⅱb期15例,Ⅲ期1例.所有患者均采用经溶栓导管局部持续溶栓的方法介入治疗.结果 16例患者中2例(12.5％)患者经介入溶栓导管溶栓后病变血管完全再通;14例(87.5％)患者病变血管部分再通,肢体得以保全.后期随访1例(6.25％)患者肢体发生不可逆性坏死而行截肢术;3例(18.8％)遗留有下肢间歇性跛行症状.结论 经溶栓导管介入溶栓治疗ALI安全、有效,部分患者肢体可得到保全.%Objective To discuss the technique and efficacy of percutaneous catheter - directed thrombolysis for the treatment of acute lower limb ischemia. Methods Sixteen patients with acute lower limb ischemia were enrolled in this study. According to Rutherford criteria, 15 cases belonged to category Ⅱb, one case belonged to category Ⅲ. Interventional percutaneous catheter - directed continuous regional perfusion thrombolysis was carried out in all patients. The clinical results were analyzed. Results Of the 16 patients, complete recanalization was obtained in 2 (12.5%) and partial recanalization in 14 (87.5%). During the follow- up period, 3 cases (18.8%) developed intermittent claudication and one case (6.25%) had to receive amputation due to irreversible limb necrosis. Conclusion Interven - tional percutaneous catheter -directed thrombolysis is a safe and effective treatment for acute lower limb ischemia with lower amputation rate.
