MR扩散加权成像对大鼠胰腺纤维化的诊断价值

         

摘要

Objective To explore the diagnostic value of magnetic resonance(MR)diffusion-weighted imaging(DWI)in rats with pancreatic fibrosis.Methods Forty-five rats were randomly divided into two groups:experimental group(n=39)and normal control group(n=6).Rats in the normal control group were intraperitoneally injected with an equal volume of 0.9% sodium chlo-ride.In the experimental group,pancreatic fibrosis was induced by intraperitoneal diethyldithio-carbamate inj ection and high fat diet using the incremental method.Thirteen rats in the experi-mental group and two rats in the control group were randomly selected for MR DWI scans after intervention for 3,4 and 5 weeks,respectively.In addition,HE and Masson staining of pancreatic tissues were performed,and the relationships of ADC and eADC values to the degree of pancreatic fibrosis were analyzed according to the pathological stages.Results Among the rats in experi-mental group,8 had S1 fibrosis,9 had S2 fibrosis,6 had S3 fibrosis,and 8 had S4 fibrosis.ADC value decreased and eADC value increased with the aggravation of pancreatic fibrosis(P<0.05). ADC value was negatively correlated with pancreatic fibrosis stage(r=-0.978,P<0.01),but eADC value was positively correlated with pancreatic fibrosis stage(r=0.983,P<0.01).Conclu-sion Pancreatic fibrosis can be induced by intraperitoneal diethyldithiocarbamate inj ection with high fat diet in rats.The values of ADC and eADC,respectively,have negative and positive correlation with the degree of fibrosis.Therefore,MR DWI has an important prognostic value in differ-ent degrees of pancreatic fibrosis.%目的 探讨磁共振(MR)扩散加权成像(DWI)对大鼠胰腺纤维化的诊断价值.方法 将45只大鼠随机分为2组:实验组(39只)和正常对照组(6只).正常对照组大鼠采用等容量的0.9%氯化钠注射液腹腔注射.实验组采用递增法腹腔注射二乙基二硫代氨基甲酸盐及高脂饮食建立大鼠胰腺纤维化模型.干预3、4、5周后随机选取实验组大鼠13只、正常对照组大鼠2只行MR DWI扫描.实验组取胰腺组织行苏木精-伊红(HE)染色、Masson染色.根据病理分期,分析各期ADC值、eADC值与胰腺纤维化程度的关系.结果 实验组大鼠胰腺纤维化分期为S1期8只,S2期9只,S3期6只,S4期8只.随着胰腺纤维化程度的加重,ADC值减低,eADC值增高,差异有统计学意义(P<0.05).ADC值与胰腺纤维化分期呈负相关(r=-0.978,P<0.01),eADC值与胰腺纤维化分期呈正相关(r=0.983,P<0.01).结论 递增法腹腔注射二乙基二硫代氨基甲酸盐配合高脂饮食能够成功诱导大鼠胰腺纤维化,ADC值、eADC值与纤维化程度分别呈负、正相关,对不同程度的胰腺纤维化具有重要的诊断价值.
