目的 探讨经鼻持续正压通气联合枸橼酸咖啡因治疗早产儿呼吸暂停的临床效果.方法 将40例早产儿呼吸暂停患儿按随机数字表法分为观察组和对照组,每组20例.观察组采用经鼻持续正压通气联合枸橼酸咖啡因治疗,对照组采用经鼻持续正压通气治疗.比较2组治疗后的临床疗效、呼吸暂停发生次数与消失时间、机械通气率及病死率,观察2组治疗期间不良反应发生情况.结果 观察组治疗总有效率显著高于对照组(90％比75％,x2 =4.492、P=0.034),而不良反应发生率、机械通气率、病死率及呼吸暂停发生次数与消失时间均显著低予对照组(均P＜0.05).结论 采用经鼻持续正压通气联合枸橼酸咖啡因对早产儿呼吸暂停治疗具有较好的效果,并且治疗后不良反应更少、呼吸暂停发生率更低,在临床上具有较高的应用价值.%Objective To explore the clinical efficacy of nasal continuous positive airway pressure(NCPAP) combined with caffeine citrate in the treatment of apnea in premature infants.Methods Forty premature infants with apnea were randomly treated with NCPAP alone(control group,n=20) or in combination with caffeine citrate(observation group,n=20).Clinical efficacy,apnea frequency,apnea disappearance time,mechanical ventilation rate,mortality rate and adverse reactions were observed in both groups.Results The total effective rate in observation group was significantly higher than that in control group(90％ vs 75％,x2 =4.492,P=0.034).In addition,compared with control group,the adverse reactions,mechanical ventilation rate,mortality rate,apnea frequency and apnea disappearance time obviously decreased in observation group(all P＜ 0.05).Conclusion NCPAP combined with caffeine citrate is effective for apnea and leads to less adverse reactions and lower apnea frequency in premature infants.The combined treatment is of great value in clinical application.
