双通道经皮肾镜碎石术对肾血流动力学的影响

         

摘要

目的:探讨双通道经皮肾镜碎石术对肾血流动力学的影响。方法采用彩色多普勒血流显像(CDFI)检测55例肾结石患者双通道经皮肾镜碎石术术前及术后1、3个月肾主动脉、段间动脉、叶间动脉收缩期峰值流速(Vmax),收缩期峰值流速(S)与舒张期末流速(D)比值(S/D)和阻力指数(RI)。其中双通道位于同一肾段22例(A组),位于不同肾段23例(B 组)。结果55例患者术前患侧肾与健侧肾肾主动脉、段间动脉、叶间动脉的 Vmax、S/D、RI 值比较,差异均无统计学意义(均 P >0.05)。A、B 组术前肾主动脉、段间动脉、叶间动脉的 Vmax、S/D、RI值比较与术后1、3个月比较差异均无统计学意义(均 P >0.05)。结论双通道经皮肾镜碎石术在术后1、3个月对肾血流灌注无影响。%Objective To explore the effect of dual channel percutaneous nephrolithotomy on renal hemodynamics.Methods Fifty-five patients with kidney stones underwent dual channel percutaneous nephrolithotomy.Among them,dual channel percutaneous nephrolithotomy was performed in same renal segment in 22 (group A),and in different renal segments in 23 (group B).The peak systolic velocity (Vmax),systolic/diastolic velocity ratio (S/D)and resistance index (RI)of renal aorta,intersegment artery and interlobar artery were measured by color Doppler flow imaging before operation and 1 and 3 months after operation.Results There were no signifi-cant differences in Vmax,S/D and RI of renal aorta,intersegment artery and interlobar artery be-tween ipsilateral and contralateral kidneys before operation (P >0.05).In addition,no obvious changes in these parameters were found 1 and 3 months after operation in both groups (P >0.05).Conclusion Dual channel percutaneous nephrolithotomy has no effect on renal hemody-namics 1 and 3 months after operation.
