目的 分析临床研究报道的海藻玉壶汤中海藻与甘草的配伍规律与特点,以便为反药配伍的深入研究提供参考依据.方法 以"海藻玉壶汤"为关键词,在中国期刊全文数据库(1979-2010年)、中文科技期刊数据库中(1989-2010年),对文献全文进行检索,选择应用海藻玉壶汤的各种临床研究报道,且处方中含有"海藻-甘草"反药药对的文献,针对其主治病证、证候类型、随证配伍使用的中药及成方、海藻与甘草用法用量等进行数据分析.结果 检索得到海藻玉壶汤文献共计1137篇,其中含有"海藻-甘草"反药配伍的文献共计60篇,占总数的5.28%;以海藻玉壶汤为主方,随症加减用于妇科、男科、皮肤科等多种疾病,而并非单纯局限于对瘿病的治疗;其主治病证中证候类型以气滞痰凝型为主;海藻玉壶汤应用于临床,多与化痰药、活血化瘀药、理气药、清热药、补益药等配伍使用;海藻与甘草比例以2∶1~5∶1之间的较为常见,占文献总数的74.60%,均未见不良反应报道.结论 含有反药配伍的海藻玉壶汤在临床应用中范围较广;其主治病证中证候类型仍以气滞痰凝型为主,与原方应用相吻合;海藻与甘草配伍比例与原方有明显差异.%OBJECTIVE To analyze compatibility rules and characteristics of Sargassum and Radix Glycyrrhizae in clinical reports of Haizao Yuhu Decoction in order to provide reference evidence for further research of incompatible medicinal combination. METHODS The full text of literature was retrieved in CNKI (1979-2010) and VIP (1989-2010) databases by taking "Haizao Yuhu Decoction" as the keyword. Various clinical research reports applied Haizai Yuhu Decoction were selected and EXCEL was used to analyze the data of literature with Sargassum-Radix Glycyrrhizae antagonistic pair in prescriptions on the basis of their indications, syndrome patterns, modified Chinese drugs and prescriptions according to syndromes, usages and dosages of Sargassum and Radix Glycyrrhizae. RESULTS 1 137 pieces of literature were all retrieved, in which 60 pieces occupying the total number of 5.28％ were the literature with Sargassum-Radix Glycyrrhizae antagonistic combination. Modified Haizao Yuhu Decoction was used for various diseases like gynecology, andrology, dermatology instead of simplified limited treatment on goiter. Qi-stagnation and phlegm-coagulation pattern was the primary syndrome pattern among indications. The clinical application of Haizao Yuhu Decoction was frequently combined with phlegm-removing drugs, drugs for promoting blood circulation to remove blood stasis, qi-regulating drugs, heat-clearing drugs and tonic drugs. The proportion of Sargassum and Radix Glycyrrhizae between 2: 1 and 5: 1 was more common in the compatibility application of clinical doctors, taking up 74.60% of the total number of literature, and there were no reports about adverse reactions. CONCLUSION Haizao Yuhu Decoction with antagonistic compatibility has a wider range in clinical application. Among its indications, the primary syndrome pattern is still qi-stagnation and phlegm-coagulation pattern and is in accordance with the application of original prescription. The compatibility proportion of sargassum and Radix Glycyrrhizae conspicuously differs from the original prescription.
