系统超声检查在不同孕期诊断胎儿畸形的临床观察

         

摘要

Objective To explore the clinical value of ultrasonography in the diagnosis of fetal malformation at different stages of gestation.Methods Five thousand and two hundred pregnant women who underwent prenatal fetal malformation detection by ultra -sonography in our hospital from January 2014 to January 2016 were selected as research subjects .In accordance with different stages of gestation, all the pregnant women received ultrasonographic screening at different stages of gestation .They were divided into the first tri-mester stage (11 to 13 +6 weeks), the second trimester stage (18 to 27 +6 weeks) and the third trimester stage (28 to 40 +6 weeks), with each trimester receiving ultrasonography , the screening results were recorded , and postpartum or autopsy results were compared . Results Prenatal ultrasonographic screening revealed that there were 160 cases of fetal malformation , and with the detection results of abortion or post partum as the gold standards , there were 168 cases of fetal malformation , with the diagnostic accuracy being 95.24%. Cases of fetal malformation detected at early , middle and late stages were respectively 26, 115 and 19 cases, accounting for 16.25%, 71.88%and 11.87%respectively, with the detection rate of fetal malformation at the second stage being the highest .Statistical signifi-cance could be noticed, when compared with those of the early and late stages (P<0.01).Thirty, 136 and 23 fetal malformations were respectively screened out at early , middle and late gestation stages , with the central nervous system and chest abdominal deformity being the main deformity at the early stage , the neck-face and cardiovascular system deformity being the main deformity at the middle stage , and the urinary system deformity and cancer being the main problems at the late stage .Statistical significance could be found in the dis-tribution ratio of fetal malformation at different stages of gestation , when comparisons were made between them (P<0.05).Conclusion Ultrasonography was of important clinical value in the prenatal diagnosis of fetal malformation at different gestational stages .Through comparison of the malformation feature at different stages , the detection rate of fetal malformation could be obviously increased , and was of important clinical significance to the bearing and rearing of better children , with this reason it was worth further clinical promotion .%目的 探讨不同孕期采用系统超声检查对胎儿畸形诊断的临床价值.方法 选取2014年6月至2016年6月来我院行产前胎儿畸形超声检查的孕妇5200例,所有产妇均在不同孕期进行超声筛查,按孕周分为孕早期(11~13+6周)、孕中期(18~27+6周)和孕晚期(28~40+6周),记录超声筛查结果,并将其与产后或尸解结果进行比较.结果 超声筛查共检出160例孕妇子宫内胎儿畸形(以引产或分娩后检查结果为标准),共168例畸形胎儿(8例未检出),诊断准确率为95.24%.孕早、中、晚期筛查出的畸形胎儿分别有26例、115例、19例,所占比例分别为16.25%、71.88%、11.87%;孕中期畸形胎儿的检出率最高,与孕早期、孕晚期比较差异有统计学意义(P<0.01).孕早期、中期、晚期分别筛查出30、136、23不同系统器官的胎儿畸形,孕早期畸形以中枢神经系统、胸腔腹壁畸形为主,孕中期以颈面部、心血管系统畸形为主,孕晚期畸形主要集中在泌尿系统、肿瘤为主,胎儿畸形在不同孕周的分布构成比差异有统计学意义(P<0.05).结论 在不同孕期胎儿畸形的产前诊断中,系统超声检查具有重要的临床价值,通过对比不同孕周超声筛查的特点,可提高畸形胎儿的检出率.
