目的：探讨翼状胬肉切除术联合自体角膜缘干细胞移植术后佩戴软性角膜接触镜对患者角膜创面上皮修复、术后复发率及眼部舒适度的影响。方法选择2014年1月－2015年11月在新疆维吾尔自治区人民医院眼科就诊的原发性单侧翼状胬肉患者68例（91只眼），翼状胬肉均为鼻侧，侵入角膜1．5～5 mm。将患者分为两组：试验组35例（角膜缘自体干细胞移植术＋软性角膜接触镜），对照组33例（角膜缘自体干细胞移植术）。术后两组患者均给予相同药物治疗。观察两组患者角膜上皮的愈合时间及术后眼部舒适度，比较两组患者术后复发率。结果试验组患者角膜上皮愈合时间为（3．10±0．34）d，对照组为（5．67±1．12）d，差异有统计学意义。术后舒适度评分：第1天试验组舒适度评分为（7．57±1．68）分；对照组舒适度评分为（14．18±2．16）分；第3天，试验组舒适度评分为（4．98±1．60）分；对照组舒适度评分为（10．23±1．91）分；第5天，试验组组舒适度评分为（3．27±1．22）分；对照组舒适度评分为（6．38±1．69）分；第7天，试验组舒适度评分为（2．10±1．12）分；对照组舒适度评分为（2．15±1．23）分，差异均有统计学意义（P ＜0．05）。术后随访3个月，试验组无复发，对照组1只眼复发，差异无统计学意义（P ＝0．4396）。结论翼状胬肉切除联合角膜缘自体干细胞移植术后使用软性角膜接触镜可以缩短患者角膜损伤上皮的修复时间，改善患者术后眼部舒适度。%Objective To investigate the effects of pterygium excision with corneal contact lens on corneal epithelia healing,comfort and postoperative recurrence rate.Methods 68 patients (91 eyes)diagnosed from January 2014 to November 2015 in People's Hospital of Xinjiang Uygur Autonomous Region with uni-lateral pterygium (nasal side)that covered cornea surface 1.5-5 mm were chosen.And the patients were then divided into two groups at random.One group (35 cases)named experimental group were treaded by excision,combined with autologous limbal stem cell transplantation and wore soft corneal contact lens and, another group (33 cases)named control group were treaded by excision and combined with autologous lim-bal stem cell transplantation only.Two groups of postoperative patients were given the same drugs.Ob-served two groups of patients with corneal epithelium completely healing time and postoperative eye com-fort and understand the two groups of patients with postoperative recurrence rate.Results There was no statistically significant of age,gender in experimental group and contrast group.The average duration of corneal epithelial wound healing was (3.10±0.34)d in experimental group and (5.67±1.12)d in contrast group,respectively.The Comfort score on the 1 day after surgery in experimental group and contrast group was (7.57±1.68)points and (14.18±2.16)points.The third day was (4.98±1.60)points and (10.23±1.91) points.The fifth day was (3.27±1.22)points and (6.38±1.69)points.The seventh day was (2.10±1.12) points and (2.15±1.23)points.There was statistically significant in comfort on the first day,the third day, the fifth day and seventh day after surgery between experimental group and contrast group (P <0.05).Pa-tients were followed up for 3 months after surgery.No one relapse in experimental group and one eye with unilateral pterygium in control group,respectively (P = 0.439 6 ).Conclusion Wearing soft corneal contact lens after ptepygium excisions is helpul to promote corneal epithelial wound healing and increase comfort in patients.
