Objective To investigate the value of serum mesothelin ,vascular endothelial growth factor(VEGF) and carbohydrate antigen 125(CA125) in the differentiation diagnosis of benign and malignant ovarian tumors .Methods The levels of serum me-sothelin ,VEGF and CA125 in 181 patients with ovarian cancer and 136 patients with benign ovarian tumors were detected and the results were performed the Pearson correlation analysis ,receiver operating characteristic(ROC) curve and Logistic regression-ROC curve analysis of the combined indicators .Results The levels of serum mesothelin ,VEGF and CA125 in the ovarian cancer group were significantly higher than those in the benign ovarian tumors group (P<0 .05) .Serum mesothelin was significantly correlated with VEGF and CA125(P<0 .05) ,the relative coefficients(r) were 0 .349 and 0 .587 respectively .The areas under the curve(AUC) of serum mesothelin ,VEGF and CA125 were 0 .81 ,0 .90 and 0 .84 respectively ,cutoff values were 5 .04 ng/mL ,57 .2 U/mL and 582 .9 pg/mL ,respectively .The AUC of logistic regression-ROC curve in combined detection of three serum biomarkers was 0 .93 [95% confidence interval(CI):0 .90-0 .96] ,at the cut-off value(probability) of 0 .65 ,the sensitivity and specificity were 89 .1%and 87 .9% respectively .The diagnosis accuracies of Logistic regression model for ovarian cancer and benign ovarian tumor were 90 .1% and 96 .3% respectively .Conclusion The combined measurement of serum mesothelin ,VEGF and CA125 is conducive to the identification of benign and malignant ovarian tumors .%目的 探讨血清间皮素、血管内皮生长因子(VEGF)与糖类抗原125(CA125)对卵巢良恶性肿瘤的鉴别诊断价值.方法 测定181例卵巢癌和136例卵巢良性肿瘤患者血清间皮素、VEGF和CA125水平,并进行Pearson相关性分析、受试者工作特征(ROC)曲线和联合指标的Logistic回归-ROC曲线分析.结果 卵巢癌组血清间皮素、VEGF和CA125水平显著高于卵巢良性肿瘤组(P<0.05).血清间皮素和VEGF水平与CA125均呈显著正相关(P<0.05),相关系数(r)分别为0.349和0.587.血清间皮素、VEGF和CA125的ROC曲线下面积(AUC)分别为0.81、0.90和0.84,诊断界值分别为5.04 ng/mL、57.2 U/mL和582.9 pg/mL.联合检测的Logistic回归-ROC曲线的AUC为0.93(95%CI:0.90~0.96),在临界值(概率)为0.65时,敏感度和特异度分别为89.1%和87.9%.联合检测血清间皮素、VEGF和CA125的Logistic回归模型对卵巢癌和卵巢良性肿瘤的诊断正确率分别为90.1%和96.3%.结论 血清间皮素、VEGF与CA125联合检测有助于卵巢良恶性肿瘤的鉴别.
