Objective To identify, evaluate and summarize the effectiveness of advanced practice nursing model on care to elderly people. Methods Several electronic databases and websites both in English and Chinese were retrieved. The studies were selected according to the inclusion and exclusion criteria and were critically appraised by two reviewers. Data of included studies were extracted and analyzed by descriptive analysis. Results Fourteen studies were included. Advanced practical nursing model for the elderly people improved conditions of deglutition disorders, ulcer, constipation and urinary incontinence, reduced the incidence of adverse events, improved the patient satisfaction, nurses′ knowledge and skills of geriatric caring, patients′ physiological and psychological function, reduced outpatient service rate and hospitalization rate, and improved the patients′quality of life. The practical nursing model applied in China was hospital-based care model, while out-hospital extending nursing models were widely implemented in foreign countries. Conclusions Different models of advanced practice nursing can effectively improve elderly patients′ health outcome. Advanced practice nursing for elderly people in China is still in the initial stage and has to develop from specialist traing, multi-department and continued nursing.%目的：分析评价国内外高级护理实践模式在老年患者中的应用效果。方法由2名研究者对高级护理实践模式在老年患者中应用效果的中文及英文献进行检索，并按照事先规定的纳入和排除标准筛选文献、提取资料、质量评价等获取文献，了解国内外高级护理实践模式在老年患者中应用效果状况。结果共纳入14项研究。高级护理实践模式在老年患者中的应用改善了患者吞咽障碍、压疮、便秘、尿失禁等问题，降低了跌倒、误吸、压疮等不良事件发生率，提高了患者满意度及护理人员老年护理知识技能，改善了老年患者生理心理功能，降低了门诊率和住院率，提高了患者生活质量。国内应用的实践模式为院内照护模式，参与人员只有护理团队；国外则以院外照护模式为主，由高级实践护士（advanced practice nurse， APN）护理团队或多学科协作团队参与。结论高级护理实践模式有效改善了老年患者的健康结局。国内外实践模式存在较大差异，与国外相比我国老年高级护理实践尚处于起步阶段，可从专业人才培养、多学科协作、开展延续护理等方面进一步推动老年护理的发展。
