Objective:To discuss the application effect of multi-discipline cooperative transition nursing in prevention of ICU post syndrome.Methods:Since the end of January 2016, our department has implemented safety and psychological support based multi-discipline cooperative transition nursing model for the patients from our department to the ordinary department, and 59 transferred patients before (July to December 2015) and after (January to June 2016) as the research target, and comparison was made on the difference of relevant observation indicators before and after implementation.Results:The incidence rate of ICU post syndrome before implementation was significantly lower than that after implementation, and the score of nursing satisfaction before implementation was significantly higher than that after implementation (P<0.05).Conclusion:Using multi-discipline cooperative transition nursing for intervention to the ICU transferred patients could reduce the risk of ICU post syndrome and increase the patients' satisfaction.%目的:探讨多学科合作式过渡期护理在ICU后综合征预防中的应用效果.方法:我科自2016年1月起开始对自我科转入普通科室的患者实施基于安全与心理支持的多学科合作式过渡期护理模式,选择实施前(2015年7~12月)和实施后(2016年1~6月)的转出患者各59例做为研究对象,比较实施前后的各相关观察指标的差异性.结果:实施前ICU后综合征发生率显著低于实施后,护理满意度评分显著高于实施后(P<0.05).结论:采用多学科合作式过渡期护理对ICU转出患者实施干预,有利于降低ICU后综合征发生风险,提高患者满意度.
