目的 探讨非脊柱及其附件源性坐骨神经痛的病因及影像特点.方法 回顾性分析经病理、临床或随访证实的非脊柱及其附件源性坐骨神经痛患者的影像及临床资料30例.结果 非脊柱及其附件源性坐骨神经痛的病因包括:肿瘤性、感染性、腰骶部椎管内病变、梨状肌综合征等,/23例,临床表现多为干性或根性坐骨神经痛;全身性疾病如:糖迷病、痛风、强直性脊柱炎等/7例,也可引起坐骨神经痛,临床表现多为根/干迷合性坐骨神经痛.结论 非脊柱及其附件源性坐骨神经痛在临床工作中并不少见,且常被误诊,影像迷查能准确做出病因诊断,为临床治疗提供可靠的依据,可以避免不自要的自效治疗.%Objective To explore the etiology and imaging features of sciatica derived from non-spine and its attachments.Methods The imaging and clinical materials in 30 patients of sciatica derived from non-spine and its attachments diagnosed on pathology and clinical sympotom were analyzed retrospectively.Results The etiology of sciatica derived from non-spine and its attachments included tumor,infection,diseases in lumbosacral spinal canal and piriformis syndrome in 23 cases,and its manifestation was nerve trunk pain or nerve root pain.Sciatica could also be caused by systemic disease like diabetes mellitus,gout and ankylosing spondylitis (7 cases),and its manifestation was mixed with nerve root pain and nerve trunk pain.Conclusion Sciatica derived from non-spine and its attachments is not rare in clinical practice which may be misdiagnosed,and comprehensive imaging evaluation is mandatory for etiology diagnosis.
