目的：探讨高通量透析器（HFHD）串联灌流器（HP）透析对尿毒症皮肤瘙痒的影响。方法将36例透析患者随机分为2组，A组（HFHD＋HP）、B组（HFHD），在试验前及试验3个月后分别对2组患者瘙痒症状进行评分，同时检测透前血肌酐（Cr）、尿素氮（BUN）、血磷、全段甲状旁腺激素（iPTH）、血浆白蛋白水平（ALB）、超敏C反应蛋白（hs－CRP）等指标并进行比较。结果 A、B组在治疗后皮肤瘙痒程度、分布范围、对睡眠干扰均有改善（P＜0．05），A组皮肤瘙痒程度、对睡眠干扰较B组减轻更为明显（P＜0．05）；A、B组在治疗3个月后血磷、iPTH、hs－CRP均明显下降（P＜0．01），A组与B组相比血磷、iPTH差异有统计学意义（P＜0．05）。结论 HFD＋HP能减轻患者的皮肤瘙痒，降低炎症指标，改善营养不良，进而可能会提高血液透析患者的生存质量。%Objective To study the effects of high flux hemodialysis (HFHD) contacted with hemope refushion (HP) on skin pruitus in patients with uremia. Methods Thirty-six patients were randomly divided into two groups,group A (HFHD+HP),group B (HFHD);before the test and three months after test the two group's patients were evaluated in itching score,received simultaneous detection of predialysis serum creatinine(Cr),blood urea nitrogen (BUN),serum phosphorus, intact parathyroid hormone (iPTH),plasma white protein level (ALB), high-sensitivity C-reactive protein (hs-CRP) and other indicators and were compared between both groups. Results GroupA,B in the degree of skin itching,distribution after treatment for sleep disturbances were all improved (P<0.05),in group A degree of skin itching,sleep disturbance was more obvious than that in group B mitigate (P<0.05);in group A,B serum phosphorus,iPTH,hs-CRP were significantly decreased (P<0.01) after 3 months of treatment;in group A group B compared with group B serum phosphorus,iPTH difference was statistically significant (P<0.05). Conclusion HFD+HP can reduce the patient's skin itching,reduce inflammatory markers,improve malnutrition, and thus may improve the quality of life of hemodialysis patients.
