目的：探讨不同血浆靶浓度瑞芬太尼对丙泊酚抑制患者喉罩置入反应半数有效浓度（ EC50）的影响。方法：选择择期全麻行腹部或四肢手术的患者96例。随机均分为 R0，R1，R2和 R3四组。靶控输注瑞芬太尼，靶浓度分别为0，1，2，3 ng/ mL，达到设定的靶浓度后开始靶控输注丙泊酚，按 Dixon＇s 序贯法确定丙泊酚的血浆靶控浓度。计算并比较不同靶浓度瑞芬太尼对丙泊酚抑制喉罩置入反应半数有效血浆靶浓度的影响。结果：R0组、R1组、R2组和R 3组丙泊酚抑制喉罩置入反应EC50及其95％ CI分别为：3.38（2.52~4.54）μg / mL，3.11（2.25~4.30）μg / mL，2.81（1.99~3.96）μg/ mL，2.63（1.89~3.66）μg/ mL。结论：靶控输注瑞芬太尼可加强丙泊酚抑制喉罩置入反应的效应，随着瑞芬太尼靶浓度的增加，丙泊酚抑制喉罩置入反应的半数有效血浆靶浓度降低。%Objective:To investigate the effect of different plasma concentrations of remifentanil on the effective concentra-tion of propofol(EC50)in the patients with laryngeal mask insertion. Methods:All 96 patients undergoing elective general an-esthesia for abdominal or limb surgery were randomly divided into four groups of R0,R1,R2 and R3. Target controlled infusion of remifentanil,the target concentrations were 0,1,2,3 ng/ mL,the target concentration was set at the beginning of target con-trolled infusion of propofol,according to the Dixon's sequential method to determine the plasma concentration of propofol. The effects of different target concentrations of remifentanil on the effective plasma target concentrations of propofol were also com-pared. Results:The inhibitory effects of EC50 and 95% CI in R0 group,R1 group,R2 group and R3 group were as follows:3. 38 (2. 52 ~ 4. 54)μg/ mL,3. 11(2. 25 ~ 4. 30)μg/ mL,2. 81(1. 99 ~ 3. 96)μg/ mL,2. 63(1. 89 ~ 3. 66)μg/ mL. Conclu-sion:Target controlled infusion remifentanil injection can strengthen propofol inhibit laryngeal mask insertion reaction effect, target controlled infusion of 0,1,2,3 ng/ mL remifentanil propofol inhibits laryngeal mask insertion reaction in the median ef-fective target plasma concentration of 3. 38 μg/ mL,3. 11 μg/ mL,2. 81 μg/ mL,2. 63 μg/ mL.
