三维适形放射治疗对犬自发性前列腺增生症前列腺体积的作用研究

         

摘要

探索三维适形放射治疗(3-DCRT)对犬自发性前列腺增生症的前列腺体积的作用.选择7~15岁老年犬自发性BPH模型.经直肠B超测量前列腺横径、前后径和长径,根据<国家新药研究指南>,前列腺三径的乘积>18 cm3者作为合格的BPH实验动物.应用3-DCRT直接照射前列腺.以90%的等剂量曲线包绕整个前列腺,单次剂量2 Gy,总剂量14 Gy,应用6MV-X线完成照射.经直肠B超测量前列腺横径、前后径和长径,乘以系数0.52,计算出前列腺体积,进行照射前、照射后1月比较.结果:照射前前列腺横径、前后径和长径分别为(3.68±0.44)、(2.65±0.39)、(2.89±0.45)cm,照射后1月分别为(3.04±0.26)、(2.13±0.22)、(2.28±0.38)cm,照射后1月显著缩小(p<0.01或0.05).照射前、照射后1月,前列腺体积分别为(14.88±5.50)、(7.80±2.45)cm3,缩小非常显著(P<0.01)说明3-DCRT可以显著缩小前列腺体积,从而可能改善尿路梗阻和LUTS.%To explore what effect three -dimensional conformal stereotactic radiation therapy (3-DCRT) of benign prostatic hyperplasia (BPH) exert on the volumn of the prostate, First,7 ± 15-year-old canines with spontaneous BPH were chosed as experimental models. the diameters of left-right ,anterior-posterior, and superior-inferior of prostate with transrectal ultrasound are measured. According to "National Drug Research Guide," the animals of which the products of three-path of prostates were > 18 cm3 qualified as experimental BPH models. Second, the prostate directly exposed to 3-DCRT. The whole prostate of per animal was the target and sorrounded by 90% isodose curve,and was subjected to irradiation of 6 MV X-ray by 2 Gy per time and the total dose of 14 Gy. Third,the indicates were compared. The prostate volumes which were measured with transrectal ultrasound ( It calculated the product of the diameters of left-right,anterior-posterior, and superior-inferior of prostate,then multiplied by the coefficient of 0.52 )were compared between before and after irradiation for 1 month. The diameters of left-right, anterior-posterior, and superior-inferior of prostate were (3.68 ± 0.44), (2.65 ± 0.39), (2.89 ± 0.45) cm respectively before irradiation,and were (3.04 ±0.26), (2.13 ±0.22), (2.28 ±0.38) cm respectively after irradiation for 1 month. The threepath of prostate all narrowed in varying degrees. Before and after irradiation for 1 month ,the the prostate volume are ( 14.88 ±5.50), (7.80 ±2.45) cm3 respectively and reduces very significantly (P <0.01 ). It is conclused that 3-DCRT can very significanfiy reduce the prostate volume thus improve urinary tract obstruction and LUTS.
