128层CT与超声在评价心肌桥-壁冠状动脉致左心功能改变中的应用

         

摘要

目的:探讨128层CT检查和超声心动图检查心肌桥-壁冠状动脉患者左室局部心肌的收缩功能和整体心功能指标相应数据相关性及其应用价值.方法:将104例心肌桥患者,按检查方法分为CT组和超声组,测量评估指标分别为:左心室收缩末期最大内径、舒张末期最大内径和短轴收缩率(FS),腱索水平室间隔(IVS)收缩末期厚度、舒张末期厚度、收缩率、运动度,腱索水平左室后壁(LVPW)收缩末期厚度、舒张末期厚度、收缩率和运动度,左心室收缩末期(ESV)、舒张末期容积(EDV)和射血分数(EF).将CT和超声各相关数进行两样本的t检验及相关性比较分析.结果:CT和超声的左心室功能各指标值具有较好的相关性,r均大于0.7.结论:128层CT与心脏超声在评价心肌桥患者腱索水平左心室肌段的收缩功能、EDV、ESV和EF等指标具有较好的一致性,可将CT作为客观评价该类患者常用的检查手段之一.在CT组和超声组腱索水平IVS运动度、LVPW收缩末期厚度和EF各自的相关性显著高于其他指标.%Objective: To investigate the index correlation of the systolic function of left ventricular myocardium and global cardiac function in patients with myocardial bridge and wall coronary artery examined by 128-slice CT and echocardiography.Methods: According to the checking method, 104 cases of myocardial bridge were divided into CT group and ultrasonic group.Measuring indivators were as follows: the left ventricular maximum diameter of end-systolic and end-diastolic, the short axis shrinkage(FS);the interventricular septum(IVS) end-systolic thickness, end-diastolic thickness, shrinkage and sports for the chordae tendineae level;the left ventricular posterior wall end-systolic thickness, end-diastolic thickness, shrinkage and sports for the chordae tendineae level;left ventricular end-systolic(ESV), left ventricular end-diastolic(ESV), and the ejetion fraction(EF).The databases of two groups were compared each other by two sample test and correlation comparison analysis.Results: The left ventricular function indexes of CT and ultrasonic had good correlation,were greater than 0.7.Conclusion: By using layer 128 CT and echocardiography to evaulate the myocardial bridging patients,the indexs of left ventricular systolic function in chordae level, EDV, ESV and EF have a good consistency.CT can be used as an objective evaluation of such patients commonly used one of the means of inspection.The correlation between IVS motion, LVPW end-systolic thickness and EF in the CT group and ultrasound group are significantly higher than other indexes.
