Objective :To observe changes of blood lipids ,transaminase and other related parameters , and to investigate actions of inflammatory factors on the occurrence of nonalcoholic fatty liver in obese children . Methods :A total of 121 obese children and 31 healthycontrols were enrolled in this study .Obesity was defined as a body mass index exceeding the 95thpercentile for the patients'age and sex .Nonalcoholic fatty liver was diagnosed via ultrasonographic examin ‐ation .Obese children were divided into two groups ,fatty liver group(C group) and non‐fatty liver group(B group) ,31 healthy children as control group(A group) .The height ,weight ,BMI ,waistline , hipline ,waist‐to‐hip ratio and Waist to height ratio were measured and documented for every child .Blood lipids , transaminase ,hs‐CRP ,adiponectin and TNF‐α were also measured .Results : There are significant differences in BMI ,waistline and waist‐to‐hip ratio between any two groups(P< 0 .05) .Waist to height ratio ,TG and TCH was different significantly between B /A ,and C /A(P< 0 .05) .AST and ALT were also found different significantly be‐tween any two groups(P< 0 .05) ,and levels of transaminases increased gradually among A ,B and C groups .Con‐clusion :In this study ,levels of AST and ALT were found higher than controls in early phase of nonalcoholic fatty liver in obese children .Enhancement of inflammatory reaction was in relation with fatty liver .%目的：观察单纯性肥胖儿童中脂肪肝的发生情况，以及血脂、转氨酶等指标的改变，并探究炎性因子在肥胖脂肪肝发生中的作用。方法：将121例单纯性肥胖儿童根据影像学检查结果分为脂肪肝组和非脂肪肝组，并抽取31例健康儿童为对照组。测量所有儿童的身高、体重、腰围、臀围，计算 BMI 值、腰臀比、腰围身高比。检测血脂，转氨酶及 hs‐CRP ，脂联素，TNF‐α的水平。结果：三组儿童的 BMI 、腰围、腰臀比均有显著性差异（ P ＜0．05），脂肪肝组及非脂肪肝组儿童腰围／身高比（WHtR）与对照组相比有显著性差异（ P ＜0．05），脂肪肝组及非脂肪肝组儿童 TG 及TCH 与对照组比较有显著性差异（P ＜0．05）；三组儿童的 AST 及 ALT 均有显著性差异（P ＜0．05）。结论：谷丙及谷草转氨酶在肥胖脂肪肝早期会显著升高，脂肪肝形成与炎症反应增强有关。
