目的 探讨延续护理手册对经外周静脉穿刺中心静脉置管(PICC)晚期肿瘤患者的应用效果及对患者心理状况的影响.方法 收集我院均行PICC治疗,晚期肿瘤患者94例,按患者就诊顺序分为2组,每组47例,2组患者住院期间的基础护理及药物干预相同,对照组出院后采用常规护理措施;观察组采用延续性护理同时配合PICC延续护理手册,采用Zung 焦虑自评量表(SAS)、抑郁自评量表(SDS)、匹兹堡睡眠指数量表(PSQI)来评定心理指标,比较2组间满意度,记录置管期间并发症状况.结果 治疗后对照组和观察组SAS评分分别为(51±6)、(46±5),SDS评分分别为(54±7)、(47±6),PSQI评分分别为(9±3)、(6±3),2组差异有统计学意义(P <0.05).对照组满意率83% 低于观察组满意率(96%),差异有统计学意义(P <0.05).对照组导管脱出率、局部感染率(19%、13%)高于观察组(4%、2%),差异有统计学意义(P <0.05).结论 延续护理手册对PICC晚期肿瘤患者的应用效果确切,有助于提高护理满意度,降低不良情绪和置管并发症.%Objective To explore the effect of the application of PICC Continuous Nursing Handbook on psychological status of patients with advanced cancer.Methods Ninety-four cases of advanced cancer patients in our hospital underwent PICC catheter treatment were collected,and randomly divided into 2 groups,47 cases in each.The 2 groups of patients received basic nursing and medicine intervention,and the control group received continuity of care,and the study group received more with PICC Continuous Nursing Handbook nursing,scores of self-rating anxiety scale(SAS),self-rating depression scale(SDS),Pittsburgh sleep quality index(PSQI)were re-corded to assess the psychology and satisfaction degree,and complications during catheterization were recorded. Results After the treatment,the scores of SAS in the control group and the study group were(51 ± 6),(46 ± 5), SDS scores were(54 ± 7),(47 ± 6),PSQI scores were(9 ± 3),(6 ± 3),respectively and the differences were sig-nificant(P < 0.05).The satisfaction rate of the control group(83%)was lower than that of the study group (96%)(P <0.05).The catheter withdrawal rate and local infection rate(19%,13%)in the control group were higher than those of the study group(4%,2%),with statistical difference(P<0.05).Conclusion The applica-tion of PICC Continuous Nursing Handbook in patients with advanced cancer is effective,which can improve nurs-ing satisfaction,reduce bad mood and catheter complications.
