经口间歇管饲对脑卒中后吞咽障碍患者的营养状态及生活质量的影响

         

摘要

Objective To investigate the effect of intermittent oro-esophageal intubation feeding on the nutritional status and social image of patients with dysphagia after stroke.Methods A total of 72 stroke patients with dysphagia who were admitted to our hospital during July 2014 to July 2016 were selected and numbered according to the order of admission.By the random number table method,they were divided into the observation group and the control group with 36 cases in each group.The control group was given nutritional support with persistent esophageal intubation feeding (PNG) while the observation group was given nutritional support with intermittent oro-esophageal intubation feeding.Changes of scores of Water swallow test and severity of dysphagia (VGF),the triceps skinfold thickness (TSF),the unaffected upper mid-arm muscle circumference (AMC),hemoglobin (Hb),serum albumin (ALB) and serum albumin (PA) levels on the first day of admission and on the 30th day after admission were compared between the two groups.The quality of life scores was also compared between the two groups at discharge.Results Thirty days after admission,the score of Water swallow test in the observation group was significantly lower than that in the control group (P <0.05),and the VGF score,levels of Hb,ALB and PA in the observation group were significantly higher than those in the control group (P <0.05).The scores of physiological function,physiological role,body pain,general health,social function,emotional function,mental health and total score of quality of life in the observation group were significantly higher than those in the control group (P <0.05).Conclusion The application of intermittent oro-esophageal intubation feeding in patients with dysphagia after stroke can reduce the suffering of patients,give patients adequate nutritional support,and can restore their swallowing ability as soon as possible.It also can improve the quality of life of patients.%目的 研究经口间歇管饲对脑卒中后吞咽障碍患者的营养状态及生活质量的影响.方法 选取我院2014年7月至2016年7月脑卒中后吞咽障碍的患者72例,按入院顺序编号,采用随机数字表法将患者分为观察组与对照组2组,每组36例,给予对照组持续性管饲胃肠营养法(PNG)营养支持,给予观察组经口间歇管饲营养支持,比较2组患者入院第1天和第30天时洼田饮水实验、吞咽障碍程度评分(VGF)以及肱三头肌皮褶厚度(TSF)、健侧上臂中部肌围(AMC)、血红蛋白(Hb)、血清白蛋白(ALB)、血清前白蛋白(PA)水平变化情况,比较2组出院时生活质量评分.结果 入院30 d后观察组患者洼田饮水实验评分较对照组显著较低(P<0.05);观察组VGF评分较对照组显著较高(P<0.05);入院30 d后观察组患者Hb、ALB、PA水平较对照组显著较高(P<0.05);观察组患者生理功能、生理职能、躯体疼痛、总体健康、社会功能、情感职能、精神健康等生活质量维度评分及生活质量总评分较对照组显著较高(P <0.05).结论 给予脑卒中后吞咽障碍患者经口间歇管饲能减轻患者痛苦,使其获得足够的营养支持,尽早恢复吞咽能力,提高其生存质量,值得临床进一步研究和推广.
