Objective To observe the clinical efficacy of using cobalt-chromium alloy cast post and fiber post to treat wedge-shaped defects of severe anterior teeth, premolars to prevent crown fracture. Methods 131 patients ( 282 teeth ) who had severe exposed pulp wedge-shaped defect were studied, and they were divided into A, B groups radomly. Patients in group A ( 109 teeth ) were treated with cast cobalt-chromium alloy post, cobalt-chromium alloy handcuffed ceramic crowns; Patients in group B ( 173 teeth ) were treated with glass fiber post, cobalt-chromium alloy handcuffed ceramic crowns. Results Follow-ups for 1 ~5 years showed that the success rates of two groups were 97/109,169/173. Conclusions Glass fiber post and dentin have similar elastic modulus. It 's suited for severe dental wedge-shaped defects, which can effectively prevent the crown fracture, and it is better than the cobalt-chromium alloy cast posts. Cobalt-chromium alloy has relatively higher elastic modulus, and a few cases of root fracture may occur.%目的 铸造钴铬合金桩、纤维桩运用于重度楔状缺损前牙、前磨牙,防止冠折发生的疗效观察.方法 选择已露髓的重度楔状缺损病例131例、282颗楔状缺损患牙进行了研究,分为A、B2组.A组109颗重度楔状缺损牙,采用铸造钻铬合金桩、钴铬合金烤瓷冠修复;B组173颗重度楔状缺损牙,采用玻璃纤维桩、钴铬合金烤瓷冠修复.结果 随访1~5年,A组成功97例,失败12例;B组成功169例,失败4例.结论 玻璃纤维桩与牙本质的弹性模量比较接近,用于重度楔状缺损牙,能有效地防止冠、根折的发生,效果优于铸造钴铬合金桩,铸造钴铬合金桩弹性模量相对较大,少数病例可发生根折.
