急性脑梗死患者卒中亚型与血清高敏C反应蛋白和糖化血红蛋白的关系

         

摘要

目的 探讨急性缺血性脑卒中的SSS-TOAST分型中的各个亚型与高敏C反应蛋白(hs-CRP)和糖化血红蛋白(HbA1C)的相关性.方法 选取急性缺血性脑卒中患者170例,根据SSS-TOAST分型标准分为大动脉粥样硬化性脑卒中(Large artery atherosclerosis,LAA)、心源性脑栓塞(Cardio-aortic embolism,CE)、小动脉闭塞性脑卒中(Small artery occlusion,SAO)、其他确定病因引起的缺血性脑卒中(Stroke of other demonstrated etiology,SOE)和原因不明的缺血性脑卒中(Stroke of undemonstrated etiology,SUE)5个亚型,同时测定各个亚型患者血液中hs-CRP和HbA1C的浓度.结果 大动脉粥样硬化性脑卒中(LAA)和原因不明的缺血性脑卒中(SUE)患者血中的hs-CRP的浓度高于其余3组(P<0.05),且LAA又高于SOE(P<0.05);小动脉闭塞性脑卒中(SAO)患者血中的HbA1C的浓度高于其余4组(P<0.05).结论 hs-CRP的浓度可能与动脉粥样硬化斑块的不稳定性相关,HbA1C的浓度可能与小血管病变相关,测定hs-CRP和HbA1C的浓度有助于SSS-TOAST分型的研究.%Objective To investigate the changes of serum hypersensitive C-reactive protein (hs-CRP), Glycated hemoglobin (HbA1C) in patients with acute cerebral infarction and the relationship between them and stroke subtypes of TOAST. Methods According to SSS-TOAST, 170 patients with acute cerebral infarction were divided into five groups: Large artery atherosclerosis group (LAA)、Cardio-aortic embolism group (CE)、 Small artery occlusion group (SAO)、Stroke of other demonstrated etiology group (SOE) and Stroke of unde-monstrated etiology group (SUE). The levels of serum hs-CRP and HbA1C were detected. Results In the LAA group and SUE group,the levels of serum hs-CRP were higher than those in other groups (P<0.05), and meanwhile the serum hs-CRP level in LAA group was higher than it in SUE group (P<0.05). In the SAO group,the level of serum HbA1C was higher than it in other groups (P<0.05). Conclusions The level of serum hs-CRP is closely correlated to the unstable plaque of atherosclerosis, and the HbA1C level is closely correlated to the small artery occlusion. The detection of hs-CRP and HbAlC could benefit the determination of TOAST subtypes.
