磁敏感加权成像对儿童轻型颅脑损伤微出血的诊断

         

摘要

目的 探讨磁敏感加权成像技术(SWI)对儿童轻型颅脑损伤微出血的诊断价值.方法 对28例儿童轻型颅脑损伤行常规MR和SWI检查,双盲法比较常规MR序列和SWI序列对轻型颅脑损伤微出血灶检出的差异.结果 28例中,MR常规序列检出微出血灶14例(50.00%),SWI序列检出微出血灶20例(71.43%),两者经统计学处理有显著性差异.出血灶多位于皮髓质交界处,单发病灶14例,多发6例,3例见内板下条线状硬膜下血肿.结论 SWI较常规MRI能更敏感地显示微出血灶,避免部分病灶的漏诊,建议作为儿童轻型颅脑损伤检查常规序列应用.%Objective To evaluate the diagnostic value of Susceptibility Weighted Imaging(SWI)in mild traumatic brain injury(MTBI)of children. Methods 28 cases of children with MTBI underwent routine magnetic resonance imaging(MRI)and SWI examination, and the difference of conventional MR and SWI sequence on detection of MTBI was compared and analyzed by double blind methods. Results In 28 cases,14 cases of microbleeding lesions were detected by conventional MR(50.00%)and 20 cases by SWI(71.43%). It had statistical difference on detecting microbleeding lesions between conventional MRI and SWI. Microbleeding lesions mostly located at the junctional zone of gray and white matter. The number of simple and multiple microbleeding lesions were 14 and 6 cases,and there were 3 cases with subdural hemorrage. Conclusion Compared with conventional MR,SWI can better demonstrate microbleeding lesions and avoid the missed diagnosis. It can be used as the routine sequence of MRI examination for MTBI of children.
