ObjectiveTo explore the application value of spleen-preserving distal pancreatectomy(SPDP)with splenic vessels preservation for benign pancreatic lesions.Methods68 patients who were treated by SPDP in our hospital from 2013.1 to 2015.12 were randomly divided into two groups. Group A(n=34)was treated with SPDP with splenic vessels preservation,Group B(n=34)was treated with SPDP without splenic vessels preservation. The operation related indicators,change of blood platelet count and postoperative complications were compared. Result:Compared with Group B,the operation time increased,but the hoshospital stay shortened,intraoperative blood loss reduced in Group A(P<0.05). The blood platelet count at 3,7,14 days after surgery in Group A was significantly lower than that in Group B(P<0.05). The total incidence of postoperative complications in Group A was significantly lower than that in Group B(P<0.05).ConclusionsSPDP with splenic vessels preservation for benign pancreatic lesions can uttermost preserved spleen blood supply and function,reduce postoperative complications.%目的：探讨保留脾血管的保脾胰体尾切除术（SPDP）在胰体尾良性病变中的应用价值。方法2013年1月至2015年12月行胰体尾切除术患者68例，分别采用保留脾血管的SPDP（A组，34例）与切除脾血管的SPDP（B组，21例）。比较两组手术相关指标、血小板计数变化及术后并发症。结果与B组比较，A组患者的手术时间延长，但术后住院时间明显缩短，术中出血量明显减少（P<0.05）；A组术后第3、7、14天血小板计数均显著低于B组（P<0.05）；A组术后并发症总发生率明显低于B组（P<0.05）。结论保留脾血管的SPDP治疗胰体尾良性病变可最大限度保留脾脏血供与功能，减少术后并发症。
